Zusammenfassung
Die Wirkung von Insulin bzw. Insulin und Glucose auf Coronardurchblutung, Sauerstoffaufnahme, Kohlendioxydabgabe und respiratorischen Quotienten des menschlichen Herzens wurde an insgesamt 34 Patienten im Rahmen diagnostischer Herzkatheterisationen untersucht.
Eine intravenöse Injektion von Insulin (0,2 E/kg) führt zu einer Abnahme der arterio-coronarvenösen Sauerstoffdifferenz und der prozentualen O2-Extraktion bei unverändertem arteriellen Sauerstoffangebot. Die Coronardurchblutung nimmt deutlich zu, so daß die Sauerstoffutilisation pro Minute eine geringe Erhöhung und pro Schlag bei gesteigerter Herzfrequenz keine wesentliche Veränderung erfährt. Diese Befunde lassen sich ebenso wie die Abnahme der arteriellen Kohlendioxydkonzentration mit einer sekundären Adrenalinausschüttung infolge starken Blutzuckerabfalles erklären. Die relativ zur verminderten Sauerstoffextraktion erhöhte coronarvenös-arterielle Kohlendioxyddifferenz bewirkt eine signifikante Steigerung des respiratorischen Quotienten und bei gleichzeitiger Coronardurchblutungssteigerung eine Erhöhung der CO2-Produktion pro Minute und Schlag. Diese Veränderungen werden als direkte Insulineffekte aufgefaßt und weisen auf eine erhöhte myokardiale Kohlenhydratutilisation hin.
Unter dem Einfluß von Insulin (0,1 E/kg) und Glucose (1 g/min) wird entsprechend der Verminderung des arteriellen Sauerstoffgehaltes, die durch den hyperosmolaren Effekt der zugeführten Glucose zustande kommt, vom Herzmuskel weniger Sauerstoff extrahiert. Die Sauerstoffutilisation pro Minute und Schlag bleibt jedoch wegen der kompensatorischen Coronardurchblutungssteigerung unverändert. Aus der relativen Erhöhung der coronarvenös-arteriellen Kohlendioxyddifferenz resultieren bei konstanter arterieller CO2-Konzentration eine Zunahme der CO2-Produktion pro Schlag und Minute sowie eine signifikante Steigerung des respiratorischen Quotienten.
Somit fassen wir nur den relativen Anstieg der myokardialen CO2-Abgabe und die Steigerung des respiratorischen Quotienten als direkte Insulinwirkung auf. Die übrigen mitgeteilten Veränderungen, wie beispielsweise die Zunahme der Coronardurchblutung und der Sauerstoffutilisation, lassen sich auf sekundäre Effekte zurückführen.
Summary
The effect of insulin respectively insulin and glucose on coronary blood flow, oxygen uptake, carbon dioxide release and on the respiratory quotient of the human heart was determined in 34 patients during diagnostic heart catheterisations.
Intravenous injection of insulin (0,2 U/kg) reduces the arterio-coronaryvenous oxygen difference and the percentual O2-extraction whereas the arterial oxygen level remains unchanged. The coronary blood flow increases markedly, so that the oxygen utilisation per minute is slightly raised while the O2-utilisation per beat with accelerated heart rate shows no important changes. These findings as well as the decrease of the arterial carbon dioxide concentration are explained by a secondary adrenalin release due to a substantial drop in the blood sugar level. In relation to the reduced oxygen extraction the raised coronaryvenousarterial carbon dioxide difference evokes a significant increase of the respiratory quotient and with simultaneous rise of the coronary blood flow an elevation of the CO2-production per minute and per heart beat. These changes are considered direct effects of insulin and reflect an increased myocardial carbohydrate utilisation.
Under the influence of insulin (0,1 U/kg) and glucose (1 g/min) a diminished oxygen extraction parallel to the decrease in arterial oxygen content takes place which is due to the hyperosmolaric effect of the glucose administered. The oxygen utilisation per minute and heart beat remains unchanged however, owing to the compensatory elevation in coronary blood flow. An increase of CO2-production per heart beat and minute and a significant increase of the respiratory quotient result from the relative rise of the coronaryvenous-arterial carbon dioxide difference. The arterial CO2-concentration shows no alterations.
We therefore consider the relative rise of myocardial carbon dioxide output and the increase of the respiratory quotient direct insulin effects. The other changes reported above such as the increased coronary blood flow and the rise of oxygen utilisation may be attributed to secondary effects.
