Zusammenfassung
Aus Beobachtungen der Ausscheidungen aller Hämpräkursoren im Harn beKranken mit akuter Porphyrie, sowie aus den Isoi merenverhältnissen wird wahrscheinlich, daß zwischen PBG und Uroporphyrin ein relativer Block, also ein Defekt der PBG-Deaminase, besteht. Diese Folgerung wird durch einen Inkubationsversuch mit einem aus der Leber eines Nichtporphyrinkranken und eines Falles von akuter Porphyrie gewonnenen Gewebezylinderhomogenats unter Zusatz von Delta-ALS gesichert. Demnach besteht bei der akuten Porphyrie ein relativer Block zwishenc PBG und Uroporphyrin. Dieser Block führt (regulatorisch?) zu einer gesteigerten Anflutung von Vorstufen, d.h. zu gesteigerter Bildung von Delta-ALS und PBG, wodurch der Block überwunden werden kann. Vielleicht beruht diese gesteigerte Bildung der ersten beiden Vorstufen auf der Aufhebung eines Depressormechanismus nachJacob u.Monod, wobei möglicherweise Lebersteroide nach den vonGramick u.Kappas (1967, 1968) entwickelten Vorstellungen eine Rolle spielen. Unser Befund steht im Widerspruch zu den Ergebnissen vonKiku Natao et al., welche eine erhöhte Porphyrinsynthese in einem Leberstückchen von akuter Porphyrie fanden. Die Autoren haben jedoch die einzelnen Porphyrine nicht bestimmt, ebensowenig ihre Isomerenverteilung. Sie haben nur die Lichtabsorption bei 407µµ gemessen. Unsere klinischen und experimentellen Befunde sprechen eindeutig für eine Hemmung der Porphobilinogendeaminase. Die Überproduktion von Delta-ALS ist wahrscheinlich sekundär.
Summary
It is demonstrated by urinalysis of cases of acute porphyria that relatively to the large amount of excreted Delta-ALA and porphobilinogen a relatively small quantity of porphyrin is ascertained. This creates very likely a block between porphybilinogen and uroporphyrin.
In addition, it is demonstrated that uroporphyrin in urine develops spontaneously from porphobilinogen, even if the specific ferments do not exist.
Finally it is demonstrated in a test in vitro that liver homogenat of one case of acute porphyria is capable to produce merely slight traces of porphyrin, whereas under the same circumstances by the help of liver biopsy a considerable porphyrin formation is evident.
