Zusammenfassung
Nachdem in früheren Arbeiten über Art und Eigenschaften menschlicher Leberproteine berichtet wurde, wird jetzt das Auftreten und Verhalten dieser Proteine im Blutserum bei Lebererkrankungen untersucht. Bei immunochemischen Analysen von 114 Seren Leberkranker und lebergesunder Kontrollpersonen konnte mit hoher Spezifität festgestellt werden, daß bei Lebererkrankungen Leberproteine in die Blutbahn ausgeschwemmt und im peripheren Venenblut nachweisbar werden. Diese Befunde werden für verschiedene Lebererkrankungen gesondert dargestellt. Der Nachweis von Leberproteinen im Venenblut gelingt am häufigsten im Leberkoma, weniger häufig bei akuten und seltener bei chronischen Lebererkrankungen und scheint somit an das Ausmaß jeweils vorliegender florider Leberzellschädigungen gebunden zu sein. Die Befunde werden mit einer Reihe anderer Leberparameter verglichen und auch an Verlaufskontrollen bei mehreren Patienten aufgezeigt. Es konnte dabei eine Übereinstimmung unserer Befunde mit dem klinischen Bild festgestellt werden. Abschließend werden die Ergebnisse im Zusammenhang mit den heute vorherrschenden Theorien über die Autoimmungenese chronischer Lebererkrankungen diskutiert und ihre Bedeutung für diese Anschauung erörtert.
Summary
In former studies several proteins of the human liver have been charakterized by immunochemical methods. This paper deals with the possible importance of these proteins for autoimmunisation in liver diseases. It was suggested that liver proteins which might have contact with the antibody producing cells during liver disease could represent the still unknown autoantigen. That will be true of all liver proteins which penetrate during liver parenchymal damage into the blood circulation. In order to verify this theory we made series of experiments on 114 individuals with and without liver diseases to detect liver proteins in venous blood samples by immunochemical analysis. Liver proteins have been found most frequently in cases with acute necrosis of liver parenchym and in obstructive jaundice with liver damage; less frequently in acute hepatitis, fatty liver and liver cirrhosis. The appearance of liver proteins in the blood seems to be related to the degree of liver parenchymal damage. It has been found that not all of the known liver proteins but only a few of them appear in the blood (liver protein Nr. 5, 8 and the so-called X-protein). One of them is an esterase (Nr. 8), another is a lipoprotein (X-protein). All these findings are compared with other laboratory data such as SGPT, bilirubin, electrophoresis, Takata etc. By several patients with acute hepatitis the progress of the immunochemical data has been recorded throughout the progression of the disease. The experiments verified that liver proteins have contact with antibody producing cells during liver damage due to their penetration into the bloodstream.
