Summary
Two cases of operative evacuation of pontine haematomas are described in addition to 22 cases reported in the literature. Clinical and neuroradiological findings indicate a brain stem lesion sometimes considered as an inoperable brain stem glioma. Computer assisted tomography todate easily reveals such hematomas.
Successful operative results are generally obtained. This diagnosis must be considered when the onset of symptoms is progressive and young people are affected.
We describe two young patients with rapidly progressive symptoms. In one case (case 2) the symptoms started after a bee sting but we were unable to find any real correlation. Surgical removal of blood clot was followed by progressive improvement in the patients who were able to return to normal active life.
Zusammenfassung
Zwei Fälle einer operativen Entleerung von Hämatomen im Brückengebiet werden beschrieben aufgrund von 22 Fällen aus der Literatur. Die klinischen und neuroradiologischen Befunde weisen auf eine Hirnstammläsion hin, so daß sie manchmal als inoperable Hirnstammgliome erscheinen. Heute kann man sie leicht mit dem Computertomogramm diagnostizieren. Im allgemeinen sind die operativen Ergebnisse erfolgreich. Wenn sich Symptome in dieser Gegend rasch entwickeln und junge Menschen davon betroffen werden, muß man immer an eine solche pathologische Ursache denken. Zwei Patienten werden beschrieben, bei denen diese Symptome in rascher Progression auftraten. In einem Fall (Fall 2) entstanden die Symptome nach einem Bienenstich, allerdings konnten wir keinen ursächlichen Zusammenhang feststellen. Die chirurgische Entleerung des Hämatoms brachte eine rasche Besserung im Befinden der Patienten, die zu einem aktiven Leben zurückkehren konnten.
