Zusammenfassung
Die Bestimmung der renalen Elektrolytausscheidung bei 8 Patienten mit seitenunterschiedlich ausgeprägter Pyelonephritis unter Wasser- und osmotischer Diurese hatte unter besonderer Berücksichtigung des Natrium folgendes Ergebnis:

Pyelonephritiskranke mit Hypertonie (Gruppe I) scheiden unter Wasser- und osmotischer Diurese absolut und in bezug auf das filtrierte loadmehr Natrium aus als normotone Patienten (Gruppe II) der gleichen Erkrankung.

Die Niere mit dem jeweils kleineren Filtrat weist in 6 von 8 Fällen beider Gruppen sowohl die höhere Natriumkonzentration im Urin als auch die höhere Elimination in Prozent des tubulären Natriumload auf.

3.
Unter osmotischer Diurese ist dagegen der Zuwachs der Natriumausscheidung auf der Seite mit dem kleineren Filtrat geringer als auf der Gegenseite. Unter diesen Bedingungen findet somit ein Angleich der Ausscheidungsfraktionen für Natrium statt.

4.
Die dreiphasige Kaliumelimination läßt ebenfalls Gruppen- und Seitenunterschiede erkennen, deren Ursache im Verhältnis zur Natriumausscheidung diskutiert wird. Die Chloridausscheidung folgt der des Natrium in enger Korrelation.

Das ermittelte Funktionsverhalten spricht für das Zutreffen der intact nephron-hypothesis der Niereninsuffizienz (Bricker) bei der chronischen Pyelonephritis.
Summary
Studies, performed on eight patients with bilateral but side-different pyelonephritis, concerning the renal excretion of sodium, potassium and chloride, had the following results.

Hypertensive patients with pyelonephritis (group I) excreted absolutely and in relation to the filtered loadmore sodium than normotensive patients (group II) with the same disease.

The kidney with the smaller filtration rate shows in 6 of 8 cases of the two groups both the higher sodium concentration of the urine and the greater amount excreted in percent of the tubular sodium load.

Otherwise osmotic diuresis results in a smaller increase of sodium excretion in the kidney with the more reduced filtration rate. Since, both rejection fractions of sodium approximate under these conditions.

The elimination of potassium reveals similar differences between the both groups as well as between the singular kidneys. The mechanism will be discussed in relation to the sodium excretion. The chloride amount excreted is closely correlated to that of sodium.

These results are suggested to be in correspondence with the intact nephron-hypothesis of the renal insufficiency (Bricker).
