155. Komplikationen bei der Ausschälplastik der Carotisgabel

Summary

Between 1973 and 1978 145 open reconstructions of the carotid bifurcation were done on 118 patients. 6 % showed a clinical stage I, 42% a stage II and 52% a stage IIIa (PRIND). Postoperatively 7 patients had major ischemic lesions, of which 5 were permanent. The combination of stenosis and contralateral occlusion increases the morbidity up to 14 %. Persistent lesions of extracranial nerves were found in 3 patients. Transient irritation of extracranial nerves has been observed in another 29 patients. Mortality in the whole series was 0.7 %.

Zusammenfassung

Zwischen 1973 und 1978 wurden 145 offene Ausschälungen der Carotisgabel an 118 Patienten durchgeführt. 6 % boten ein Stadium I, 42 % ein Stadium II und 52 % ein Stadium IIIa (PRIND). 7mal fanden wir postoperativ eine Zunahme der zentral-nervösen Ausfälle, davon 5mal mit bleibender Defektheilung. Die Kombination Stenose + kontralateraler Verschluß zeigte ein 3fach höheres Risiko (14 %). 3 Patienten boten persistierende Hirnnervenläsionen (V, IX, XII). Bei weiteren 29 Patienten konnte für wenige Stunden eine Irritation extrakranieller Hirnnerven festgestellt werden. 1 Patient verstarb in der Klinik an einem Myokardinfarkt.
