13. Bedeutung der präoperativen cyto- und histopathologischen Tumordiagnostik

Summary

Surgical or aspiration biopsy is indicated whenever the suspicion of a tumor cannot be excluded by other diagnostic methods. According to the present literature, biopsies do not increase the patient's risk of metastases. The optimal result of surgery would be the removal of the whole tumor. The pathologist's task is to check the site and the edges of the resection, and to define the growth and metastatic spread of the tumor. Intraoperative frozen sections providing reliable diagnoses in about 90% of cases, facilitate the surgeon's decision about the definitive procedure. Histological “grading” supplies prognostic data indicative of the necessary steps in postoperative care and postclinical follow-up.

Zusammenfassung

Eine Biopsie oder Punktion ist immer dann erforderlich, wenn der Verdacht auf einen bösartigen Tumor nicht auf andere Weise ausgeschlossen werden kann. Eine Gefährdung des Patienten durch Steigerung der Metastasenquote besteht nach heute vorliegendem Schrifttum nicht. Im Idealfall soll der gesamte Tumor entfernt werden. Zu den Aufgaben des Pathologen gehört die subtile Untersuchung insbesondere der Abtragungsstellen einschließlich der Metastasen. Der intraoperative Schnellschnitt erleichtert das operative Vorgehen und liefert in ca. 90% der Fälle klare Diagnosen. Das ≫grading≪ gibt Hinweise auf die Prognose und bestimmt damit das Vorgehen in der postoperativen Nachsorge.
