Zusammenfassung
Ein seltenes, in die Kategorie der γA-Globuline einzuordnendes Kälteagglutinin wird beschrieben. Es wurde bei einem an Plasmocytom erkrankten Patienten mit γA-Paraproteinämie nachgewiesen. Das Kälteagglutinin konnte direkt als das (myelombedingte) Paraprotein identifiziert werden. Die serologischen Daten des Kälteagglutinins weichen erheblich von den in solchen Fällen üblichen Befunden ab: Der Antikörper reagierte mit einem bisher unbekannten, durch Proteasen inaktivierbaren Receptor. Bei seiner Reaktion konnte Komplementintervention nicht nachgewiesen werden.
Summary
An unusual cold agglutinin to be classified as γA-globulin is described. It was detected with a patient suffering from plasmocytoma with γA-paraproteinemia. The cold agglutinin could directly be identified as the paraprotein (formed by the myeloma). The serologic data of the cold antibody vary considerably from in such cases usual test results: The antibody reacted with an until now unknown receptor, that is inactivated by proteases. During its reaction intervention of complement could not be detected.
