Polyarteritis nodosa and HIV infection: No evidence of a direct pathogenic role of HIV

Polyarteriitis nodosa und HIV-Infektion: Kein Hinweis auf direkte pathogenetische Bedeutung von HIV

Summary

A case of polyarteritis nodosa identified by the American College of Rheumatology (ACR) 1990 criteria in a 44-year-old HIV-infected man is described. The search for cytomegalovirus, HBV and B19 parvovirus infections was negative.In situ hybridization did not reveal proviral HIV-1 DNA in a skin sample. A zidovudine-associated vasculitis was excluded. Corticosteroid therapy resolved vasculitis manifestations and was well tolerated without opportunistic infections during the 10-month follow-up period. An indirect pathogenetic role of HIV as a possible cause of vascular damage cannot be excluded in our patient.

Zusammenfassung

Bei einem 44 Jahre alten HIV-infizierten Patienten wurde nach den Kriterien des American College of Rheumatology von 1990 eine Polyarteriitis nodosa diagnostiziert. Die Suche nach Infektionen durch Cytomegalovirus, HBV und Parvovirus B19 verlief negativ. DieIn situ-Hybridisierung deckte in einer Hautprobe keine porovirale HIV-1 DNA auf. Eine Zidovudin-assoziierte Vaskulitis wurde ausgeschlossen. Die Manifestationen der Vaskulitis verschwanden mit Kortikosteroid-Therapie, die über einen Zeitraum von 10 Monaten gut vertragen wurde und nicht durch opportunistische Infektionen belastet war. Eine indirekte pathogene Rolle von HIV als mögliche Ursache der Gefäßveränderungen kann bei unserem Patienten nicht ausgeschlossen werden.
