Zusammenfassung
Durch direkte Injektion von PSP in die Harnblase mittels einer suprapubischen Blasenpunktion gelingt es, die vonCotran undKass 1 eingeführte Farbstoffmethode der Restharnbestimmung auch bei eingeschränkter Nierenfunktion zu verwenden.
Der Eingriff ist wenig schmerzhaft und weitgehend gefahrlos. Seine Indikation ist in jedem Fall von Restharnbestimmung bei nichtinfiziertem Urin gegeben. Kontraindikationen sind: Hämorrhagische Diathese, Narbenveränderungen im Unterbauch, ungenügend gefüllte Harnblase, Gravidität.
42 in beschriebener Weise durchgeführte Restharnbestimmungen ergaben eine vorzügliche Übereinstimmung mit der unmittelbar anschließend erfolgten Messung des Restharnes durch Katheterisieren der Blase: Der Korrelationskoeffizient betrug 0,98, die mittlere Differenz 29 ml.
Mit der beschriebenen PSP-Blasenpunktionsmethode sollte es in den meisten Fällen möglich sein, bei der Restharnbestimmung den Blasenkatheterismus zu vermeiden.
Summary
The determination of residual urine by intravenous injection of Phenolphthalein (PSP) as reported byCotran andKass 1 yields poor accuracy in patients suffering from impairment of renal function.
A new method avoiding this disadvantage has been devised: the PSP is injected directly into the urinary bladder by suprapubic puncture.
Applying this method to 42 patients a highly significant correlation became evident between the amount of residual urine measured by post-voiding catheterization and the amount determined by the direct dye-test described in this paper.
The direct PSP-test represents an accurate and practically safe and painless procedure. Contraindications are: hemorrhagic diathesis, pregnancy, abdominal scars and a nearly empty bladder.
This method makes it possible to determine the residual urine without running the risk of causing an other wise non-existing urinary infection. The hazard of catheterization is, therefore, no longer justified in cases where determination of the residual urine is the only purpose of the procedure.
