Zusammenfassung
Im akuten Versuch wurde die Wirkung einer ADH-Infusion auf die Ausscheidung von Natrium, Chlorid, Kalium und Harnstoff bei Wasserdiurese und Infusion von Sterofundin bei gesunden Versuchspersonen und Patienten mit Diabetes insipidus zentralis untersucht. Es wurden folgende Ergebnisse erzielt:

Bis zur Erreichung eines maximalen U/Posm nimmt die Natrium- und Chloridausscheidung signifikant ab, steigt dann aber noch während einer maximalen Harnkonzentrierung häufiger wieder deutlich an. Auch unter Berücksichtigung von Änderungen der Inulinclearance zeigt TRNa ein gleiches Verhalten. Die akute NaCl-Retention ist absolut am geringsten bei Wasserdiurese und am stärksten bei Infusion isotonischer Salzlösung.

Die Harnstoffausscheidung nimmt im Beginn einer ADH-Infusion erheblich ab, steigt noch während der maximalen Antidiurese wieder etwas an und erhöht sich auf die Ausgangswerte oder darüber erst bei Wiedereinsetzen der Diurese. Die Harnstoffretention ist bei Infusion isotoner Salzlösungen deutlich geringer als bei Wasserdiurese.

Die Kaliumausscheidung zeigt bei Patienten mit Diabetes insipidus unter der ADH-Infusion eine signifikante Erhöhung.

Die erhobenen Befunde lassen sich mit der Hypothese vereinbaren, daß die Vermehrung des medullären Natrium- und Harnstoffpools bei Antidiurese durch vorübergehend vermehrte tubuläre Natrium- und Harnstoffreabsorption zu erklären ist.
Summary
The effect of an ADH-infusion in water diuresis and infusion of sterofundin in healthy subjects and patients with diabetes insipidus centralis on the following topics was examined: excretion of sodium, chloride, potassium, and urea.
The following results were obtained:

Sodium and chloride excretion decrease significantly until a maximal U/Posm is achieved, often however, they increase again still during maximal urine concentration. Not only the sodium output but also the excretion fraction of sodium (TRNa) shows equal changes. The acute NaCl retention is absolutely lowest in water diuresis and highest in isotonic saline loading.

In the beginning of an ADH-infusion urea excretion decreases considerably, it shows some increase still during maximal antidiuresis and raises up to the initial values or above with the again increased urinary flow. With infusion of isotonic saline solution urea retention is distinctly lower than in water diuresis.

Potassium excretion during ADH-infusion shows a significant increase in patients with diabetes insipidus centralis.

The present results are consistent with the concept that in antidiuresis a temporarily increased tubular reabsorption of sodium and urea results in an increased sodium and urea content of the medullary interstitial fluid.
