Zusammenfassung
Es wurden biochemische Untersuchungen an den Blutzellen von drei Patienten im Alter von 18–24 Jahren — darunter zwei Brüder — mit familiärer Panmyelopathie Typ Fanconi durchgeführt. Alle boten hämatologisch und klinisch die typischen Symptome wie makrocytäre hämolytische Anämie, Granulocyto- und Thrombocytopenie, Mißbildungen des-Skeletes, der Augen, Reflexanomalien, Hautpigmentierungen, Kleinwuchs, Infantilismus und Hypogenitalismus. Es wurde in den Erythrocyten, Leukocyten und Blutplättchen dieser drei Kranken eine signifikante Erniedrigung derHexokinase-Aktivitäten gefunden, während alle anderen gemessenen Enzymaktivitäten der Glykolyse, des Pentosephosphat-und Tricarbonsäurecyclus, des Glutathion- und Phosphatstoffwechsels als unspezifischer Ausdruck für die Verjüngung der Blutzellpopulation erhöht waren. Die Hexokinase war nicht nur quantitativ vermindert, sondern auch qualitativ verändert, denn ihre Affinität zu ihren Substraten Glucose war auf ein Zehntel und zum Adenosintriphosphat auf ein Drittel herabgesetzt. Da die Hexokinase die begrenzende Reaktion der Glykolyse darstellt, führt ihr Mangel zu einer Drosselung der glykolytischen Durchflußraten, der Glutathionreduktion und der ATP-Bildung, wodurch die Lebensdauer der Blutzellen verkürzt wird. Es bestehen biochemische Parallelen zur physiologischen intravitalen Erythrocytenalterung und zum Hämolysemechanismus bei den renalen Anämien. In den Leukocyten- und Knochenmarkkulturen dieser drei Kranken wurden anderorts multiple spontane Chromosomenaberrationen festgestellt. Die Chromosomenanomalien sind als Folge des Enzymdefektes zu deuten. Der Hexokinasemangel könnte theoretisch auch für den latenten Diabetes mellitus des am schwersten kranken Patienten im Sinne eines „Einschleusungstörungsdiabetes“ verantwortlich gemacht werden, doch stehen Bestimmungen der Insulinaktivitäten noch aus. Bei den klinisch gesunden Eltern und Geschwistern der zwei erkrankten Brüder konnten in den Blutzellen keine Enzymdefekte nachgewiesen werden.
Summary
Biochemical determinations in blood cells of three male patients between 18 and 24 years were performed. The three patients-two of whom were brothers-suffered from familian panmyelopathie, type Fanconi. Typical symptoms were present in all cases, clinically as well as haematologically: Macrocytic haemolytic anaemia, granulocytopenia and thrombocytopenia, malformations of bones and eyes, abnormalities of tendon reflexes, skin pigmentations, dwarfism, infantilism and hypogenitalism. Activity of hexokinase was lowerd in the erythrocytes, leukocytes and blood platelets of these three patients, whereas all other enzyme activities of glycolysis, pentose phosphate shunt and tricarboxylic acid cycle, glutathione and phosphate metabolism were above normal, probably because of the higher percentage of younger blood cells in the circulation. Hexokinase was not only diminished quantitatively, but also qualitatively. Its affinity to its substrate glucose showed a 10-fold, to ATP a more than 3-fold decrease, as compared with the normal enzyme. As hexokinase is the rate-limiting enzyme in glycolysis, glycolytic flow rates, GSSG reduction and ATP formation are lowered by its deficiency, which results in a shortening of red cell life span. There are biochemical parallels with physiological in vivo erythrocyte ageing and with the mechanism of haemolysis encountered in renal anaemias.
Multiple spontaneous chromosomal aberrations were found by other investigators in the leukocyte-and bone marrow cultures of these three cases. It remains to be determined, wether these chromosomal aberrations were caused by the hexokinase deficiency.
The latter could, also theoretically, explain the latent diabetes mellitus of the most severely diseased patient, if one would assume an “incorporation-deficient diabetes”. Estimations of insulin activity are, however, not yet made.
The parents and siblings of two pateints were clinically and haematologically healthy and did not schow any blood cell enzyme defects.
