Zusammenfassung
Unter perioperative Einmalprophylaxe mit 1,5 g Cefuroxim intravenös kam es bei einem von insgesamt 66 Eingriffen am Gallensystem (vorwiegend Cholezystektomien, 50% Risikopatienten) zu einer Wundinfektion, was einer Inzidenz von 1,51% entspricht. Hinsichtlich des Zeitpunktes der Applikation (33 Patienten erhielten nach Randomliste das Antibiotikum 20 Minuten vor und 33 Patienten 20 Minuten nach Hautschnitt) ergaben sich keine signifikanten Unterschiede; die Wundinfektion trat in der Gruppe auf, in der das Antibiotikum 20 Minuten vor Hautschnitt appliziert wurde. Fünf von 66 (7,6%) operierten Patienten entwickelten infektiöse Komplikationen. In der „präoperativen“ Gruppe traten zwei Harnwegsinfektionen und o. g. Wundabszeß (Inzidenz 9,1%) auf. In der „intraoperativen“ Gruppe traten bei einem Patienten eine Harnwegsinfektion und Pneumonie und bei einem eine Harnwegsinfektion (Inzidenz 6,1%) auf. Patienten, bei denen infektiöse Komplikationen auftraten, hatten eine deutlich verlängerte Operationsdauer. Der postoperative stationäre Aufenthalt war bei Patienten mit infektiösen Komplikationen erheblich länger im Vergleich zu Patienten ohne Komplikationen. Bei intraoperativer oder präoperativer Gabe einer prophylaktischen Dosis von Cefuroxim bestehen hinsichtlich infektiöser Komplikationsraten keine statistisch signifikanten Unterschiede.
Summary
The trial was conducted with 66 patients undergoing cholecystectomy. At random, 33 patients received 1.5 g cefuroxime i.v. as a single bolus 20 minutes before surgical incision and 33 patients were injected 20 minutes after incision. The overall incidence of wound infections was 1.51%. In the preoperative group, there was the one wound infection and two patients developed urinary tract infections (incidence of infectious complications 9.1%). In the intraoperative group, one patient developed pneumonia and urinary tract infection and another had urinary tract infection. Wound infections did not occur in this group (incidence of infectious complications 6.1%). Patients with infectious complications had a longer operation time and a longer hospital stay. In conclusion, there was no statistically significant difference in infectious complications when cefuroxime prophylaxis was given preoperatively compared to intraoperative administration.
