The diagnostic challenge of postoperative acute appendicitis

Abstract

During the period 1979 to 1989, 0.1 % of all cases of acute appendicitis at our institution occurred soon after another operative procedure. The interval between primary operation and appendectomy was 5 to 31 days (mean, 14 days). Signs and symptoms did not differ from those of classical acute appendicitis. Duration of symptoms ranged from 12 hours to 8 days (mean, 2.4 days). Perforation was present in 3 patients, suppurative appendicitis in 1 patient, and acute inflammation in 4 patients. Two of the perforations were associated with abscess formation. Morbidity related to the appendiceal condition included hepatic abscesses, septic shock, and prolonged ileus. There was no mortality. Hospitalization ranged from 6 to 80 days (mean, 12.5 days).

Résumé

Entre 1979 et 1989, 0.1% de toutes les appendicites aiguës vues dans notre institution sont survenues en période postopératoire. Le délai entre l'intervention initiale et l'appendicectomie pour appendicite postopératoire allait de 5 à 31 jours (moyenne=14 jours). Les signes et symptômes ne différaient pas de ceux de l'appendicite aiguë classique. La durée des symptômes allait de 12 heures à 8 jours (moyenne=2.4 jours). Trois patients avaient une perforation, un, une appendicite suppurée, et quatre, une inflammation aiguë. Deux fois, en cas de perforation, il y avait un abcès. La morbidité était liée essentiellement à des abcès hépatiques, à un choc septique et à un iléus prolongé. La mortalité a été nulle. La durée d'hospitalisation a été de 6 à 80 jours (moyenne=12.5 jours).

Resumen

En el período 1979 a 1989 el 0.1% de los casos de apendicitis agudas tratados en nuestra institución ocurrió después de la realización de un procedimiento operatorio. El intervalo entre la operación primaria y la apendicectomía osciló entre 5 y 31 días (promedio de 14 días). Los signes y síntomas no fueron diferentes de los de la apendicitis aguda clásica. La duración de los síntomas varió entre 12 horas y 8 días (promedio de 2.4 dfas). Se présentó perforación en tres pacientes, apendicitis supurativa en uno e inflamación aguda en cuatro. Dos de las perforaciones estuvieron asociadas con formación de absceso. La morbilidad relacionada incluyó absceso, shock séptico e íleo prolongado; no hubo mortalidad. El período de hospitalización tuvo un rango de 6 a 80 días (promedio de 12.5 días).
