Pancreatitis in Northern Newfoundland and Labrador: A 20-year review of the Grenfell Regional Health Services experience

Abstract

A retrospective review of the Grenfell Regional Health Services experience with pancreatitis over a 20-year period (1968–1987) has been presented. The small number of cases in this series precludes meaningful statistical analysis, however, in general, one can state that native peoples are represented in this study in roughly the same proportion as they are represented in the population served. We have demonstrated that pancreatitis in its chronic form does occur in the Inuit of Northern Labrador. Given the prominent role that alcohol abuse plays in the social and behavioral ills of Labrador's native peoples as reflected in the high incidence of family violence, accidental deaths, and suicides, one might anticipate a disproportionately high incidence of pancreatitis and its chronic sequelae among the Inuit [1, 2]. The answer to this seeming paradox may be found through future study of the nature of the alcohol consumed, its pattern of consumption or of genetically-determined differences in the metabolism of alcohol and its toxic effects within the body, of differences in the composition of pancreatic secretions, or of environmental and dietary factors [3, 4].

Résumé

On présente une revue rétrospective de l'expérience des services de Santé de Grenfell (Terre Neuve) sur 20 ans (1968–1987 inclus). Compte tenu du petit nombre de patients, une analyse statistique n'a pas de sens. Mais on peut dire que la proportion d'indigènes atteints est la même que dans nos régions. Nous avons démontré que la pancréatite chronique existe aussi dans l'Inuit du Labrador du Nord. Vu le rôle de l'alcool dans les fléaux sociaux et les troubles de comportement de cette population indigène et compte tenu de l'incidence extrêmement fréquente des violences familiales, des morts et des suicides, on s'attendrait à une forte incidence de pancréatite et de ses conséquences chroniques parmi la population Inuit [1, 2]. On trouvera peut-être l'explication de ce paradoxe dans l'étude future de la nature de l'alcool consommé, des différences génétiques dans le métabolisme de l'alcool et de ses effets toxiques dans le corps, des différences de composition du suc pancréatique et des facteurs d'environnement et de diététique [3,4].

Resumen

Se hace una revisión retrospectiva de la experiencia de los Servicios Regionales de Salud de Grenfell (Canada) con pancreatitis en un período de 20 años, 1968 a 1987 inclusive. El reducido numéro de casos impide un análisis estadístico significativo. Sin embargo, se puede afirmar que los nativos aparecen representados en la serie en la misma proporción que en la población general. Hemos demostrado que la pancreatitis en su forma crónica si ocurre en los nativos de la parte norte del Labrador. Dada la importancia del efecto del abuso del alcohol sobre los males sociales y de comportamiento que afectan a los nativos del Labrador y que se reflejan en una alta incidencia de violencia familiar, muertes accidentales y suicidios, se podria predecir una elevada incidencia de pancreatitis y sus secuelas crónicas en los nativos [1, 2]. La explicación de esta aparente paradoja podria ser hallada mediante un estudio future sobre la naturaleza del alcohol que se consume, el patrón de consumo, las diferencias genéticas en el metabolismo del alcohol y sus efectos tóxicos en el organismo, las diferencias en la composition de las secreciones pancreáticas y los factores ambientales y dietéticos [3,4].
