Abstract
Surgical management of chronic pancreatitis remains a difficult problem. On the continent of Europe, the main etiology of the disease is alcoholism; thus, alcohol withdrawal is mandatory before surgical treatment. Left splanchnicectomy is no longer used. Total and left subtotal pancreatectomy are abandoned due to their high mortality rates and their severe metabolic sequelae. Distal pancreatectomy is presently reserved for the cases in which the head of the pancreas is least involved. The choice between pancreaticojejunostomy and pancreaticoduodenectomy remains debated. The former has a low postoperative mortality rate while reoperation is less frequent after the latter. Current trends in Europe are to perform more anastomoses and less resections than some years ago. Pancreaticoduodenectomy, however, retains an important role in cases with biliary or duodenal involvement. Whatever the choice of the procedure, the surgical treatment of chronic pancreatitis remains palliative and does not alter the natural course of the disease. The quality and duration of the results depend mainly on alcohol withdrawal.
Résumé
Le traitement chirurgical des pancreatites chroniques reste un probleme difficile. En Europe continentale, l'alcoolisme est la principale étiologie de la maladie, aussi le sevrage de toute intoxication ethylique est-il le préalable indispensable de tout traitement chirurgical. La splanchnicectomie gauche est actuellement tombée en désuétude. La pancréatectomie totale et la pancréatectomie subtotale ont été abandonnées en raison de leur mortalité opératoire et de leurs séquelles métaboliques. La pancréatectomie gauche est maintenant réservée aux cas où les lésions respectant la tête du pancréas. Le choix entre pancreatico-jéjunostomie et duodénopancréatectomie demeure discuté. La première de ces interventions a une mortalité plus faible tandisque le taux de ré-interventions est plus faible après la seconde. La tendance générale en Europe est actuellement de faire moins de résections et plus d'anastomoses qu'il y a quelques années. La duodénopancréatectomie cependant garde un rôle important dans les cas comportant des complications biliaires ou duodénales. Quelle que soit la technique choisie, le traitement chirurgical des pancréatites chroniques reste purement palliatif et ne modifie pas l'évolution naturelle de la maladie. La qualité et la durée des résultats sont essentiellement liés au sevrage de l'intoxication éthylique.

El tratamiento quirúrgico de la pancreatitis crónica se perpétua como un problema difícil. En el continente europeo el alcoholismo constituye la primera causa o etiologi'a de la enfermedad; por consiguiente, la abstinencia de alcohol es obligatoria antes de emprender el tratamiento quirúrgico. La esplacnicectomía izquierda cayó en desuso. Las pancreatectomías total y subtotal izquierda tienden a ser abandonadas debido a su elevada mortalidad y severas secuelas metabólicas. La pancreatectomía subtotal distal es reservada en la actualidad para aquellos casos en que la cabeza del páncreas se encuentra menos afectada. La escogencia entre una pancreaticoyeyunostomía y una pancreaticoduodenectomía sigue siendo motivo de debate. La primera se acompana de una baja tasa de mortalidad postoperatoria, en tanto que las reoperaciones son menos frecuentes con la segunda. Las tendencias actuales en Europa son hacia realizar más anastomosis y menos resecciones que hace unos años. La pancreaticoduodenectomía, sin embargo, retiene una indicacion de importancia en aquellos pacientes en quienes el colédoco o el duodeno están afectados. Cualquiera que sea la escogencia del procedimiento, el tratamiento quirúrgico de la pancreatitis crónica sigue siendo paliativo y no altera el curso natural de la enfermedad. La calidad y la duración de los resultados dependen principalmente de la abstinencia de alcohol.
