Efficacy of enoxacin in the treatment of bacterial infections of the skin with regards to photosensitization

Zur Wirksamkeit von Enoxacin bei bakteriellen Infektionen der Haut unter Berücksichtigung möglicher Lichtsensibilisierung

Summary

Thirty-four patients suffering from various skin infections caused by gram-positive and gram-negative pathogens were treated with enoxacin (400 to 800 mg/day) for 8 to 14 days. A marked abatement of clinical symptoms, accompanied by eradication of the causative pathogens was observed in all 34 patients. Enoxacin was well tolerated. The minimal erythema dose of UVA light was reduced by a mean of 45% in three of the twenty patients receiving the 800 mg daily dosage of enoxacin. No positive reactions could be provoked in a subsequently applied photopatch test, allergic reactions were not observed. These mild reactions seen in a small percentage of patients appear to indicate further specific tests to determine a possible photosensitization effect of enoxacin.

Zusammenfassung

Im Rahmen einer offenen Studie erhielten 34 Patienten mit verschiedenen durch grampositive und gramnegative Erreger hervorgerufenen Infektionen der Haut zwischen 400 mg und 800 mg Enoxacin täglich oral über einen Zeitraum von 8–14 Tagen. Bei guter Verträglichkeit zeigte sich bei allen 34 Patienten ein deutlicher Rückgang der klinischen Symptomatik und eine durch bakteriologische Untersuchungen nachgewiesene Sanierung der Infektion. Spezielle Messungen der Lichtempfindlichkeit ergaben bei drei Patienten, welche jeweils eine Tagesdosis von 800 mg Enoxacin erhielten, eine Reduktion der UVA-induzierten minimalen Erythemdosis um durchschnittlich 45%. Im daraufhin angelegten Photopatch-Test ließen sich keine positiven Reaktionen provozieren, allergische Reaktionen wurden nicht beobachtet. Diese milden Reaktionen bei einem kleinen Anteil der Patienten lassen weitere spezifische Untersuchungen zur Erfassung einer möglichen lichtsensibilisierenden Wirkung des Enoxacin angezeigt erscheinen.
