Summary
The aim of our prospective clinical study was to determine whether the combination of tobramycin plus cefotaxime is more nephrotoxic than tobramycin alone. The studies were carried out in 30 patients with serious infections and normal renal function. Groups of ten patients each received either 2 g cefotaxime or 1 mg/kg body weight tobramycin or cefotaxime and tobramycin in the same dosage every eight hours intravenously for at least seven days. Serum creatinine, creatinine clearance and alanine aminopeptidase (AAP) excretion in 24-hour urine were determined before, during and five days after the antibiotics had been discontinued. These were used as parameters for glomerulotubular injury. The plasma levels of tobramycin and cefotaxime (assayed by agar diffusion) did not differ when the drugs were given alone or in combination. None of the patients treated with cefotaxime alone showed any signs of renal damage. In contrast, tobramycin alone or in combination with cefotaxime caused an increase in urinary enzymes in all patients. This activity was a mean five to six times greater than the initial values prior to antibiotic therapy. There were no significant differences between the AAP increase during treatment with tobramycin and tobramycin plus cefotaxime. Four to five days after discontinuing antibiotic therapy, AAP activity decreased to values similar to those measured prior to therapy. In some of the patients receiving tobramycin alone or in combination with cefotaxime, increased creatinine levels, a reduction in creatinine clearance to 60 ml/min and an increase in renal enzyme excretion could be observed. Thus, treatment with high doses of cefotaxime does not seem to increase tobramycin nephrotoxicity in patients with normal renal function. The nephrotoxicity of this drug combination is obviously due to the aminoglycoside.
Zusammenfassung
In einer prospektiven klinischen Studie wurde geprüft, ob die Kombination von Cefotaxim mit Tobramycin zu häufiger auftretenden und schwereren Nierenschädigungen führt als die Behandlung mit Tobramycin allein. Die Untersuchungen wurden an 30 nierengesunden Patienten mit schweren Infektionserkrankungen durchgeführt. Je zehn Patienten erhielten i. v. täglich 3 × 2 g Cefotaxim, 3 × 1 mg/kg Tobramycin oder Cefotaxim und Tobramycin 3 × täglich in derselben Dosierung über wenigstens sieben Tage. Vor der ersten Antibiotikagabe, während und bis fünf Tage nach Absetzen des jeweiligen Antibiotikums wurden bei jedem Patienten Kreatinin im Serum und die Kreatinin-Clearance bestimmt sowie die Ausscheidung der Alanin-Aminopeptidase (AAP) im 24-Stunden-Urin als Hinweis auf eine glomerulo-tubuläre Schädigung gemessen. Die Plasmakonzentrationen von Cefotaxim und Tobramycin, mittels der Agardiffusionsmethode gemessen, waren bei allen Patienten im Mittel gleich hoch, unabhängig davon, ob das Antibiotikum allein oder in Kombination verabreicht wurde. Bei keinem der Patienten, die allein mit Cefotaxim behandelt wurden, konnte ein Hinweis auf eine Nierenschädigung festgestellt werden. Im Gegensatz dazu verursachte die Behandlung mit Trobramycin allein oder in Kombination mit Cefotaxim bei allen Patienten einen Anstieg der AAP-Ausscheidung um durchschnittlich das fünf- bis sechsfache gegenüber den Ausgangswerten vor der Behandlung. Es bestand kein signifikanter Unterschied im Anstieg der AAP-Ausscheidung zwischen den beiden Behandlungsschemen. Vier bis fünf Tage nach Absetzen der Antibiotikagabe war die AAP-Ausscheidung wieder auf ihre Ausgangswerte zurückgegangen. Bei einigen dieser Patienten konnte ein pathologischer Anstieg des Serumkreatinins, eine Reduktion der Kreatinin-Clearance unter 60 ml/min sowie eine pathologisch vermehrte Ausscheidung weiterer Enzyme im Urin festgestellt werden. Diese Befunde lassen folgern, daß die zusätzliche Gabe von hohen Dosen Cefotaxim bei Patienten mit normaler Nierenfunktion die Nephrotoxizität von Tobramycin nicht zu steigern vermag; die bei der Kombination aufgetretenen Nierenschädigungen sind allein auf die nephrotoxische Wirkung des Aminoglykosids zurückzuführen.
