Abstract
The effect of surgery on inflammation was studied in male Wistar R/A rats using the carrageenin-induced edema model. Swelling of the paw was measured in standardized arbitrary units 2, 4, and 6 hr after a subcutaneous injection of carrageenin iota in the subplantar region of the right hind limb. It was significantly depressed in rats submitted to laparotomy (5.0±0.4, 8.0±1.0, 13.7±1.9) when compared with controls simply anesthetized with ether (6.2±0.5, 15.5±1.2, 23.7±0.6) ( p <0.001 at 4 and 6 hr). This inhibition lasted for at least 24 hr and was also observed after amputation, although in these experiments, the difference between operated animals and controls was not significant. Alterations of the inflammatory cellular infiltrate were studied using polyurethane sponges soaked with carrageenin lambda implanted subcutaneously in control animals and rats undergoing laparotomy or amputation. The total number of cells recovered from these sponges 5 hr after implantation was smaller in operated rats (2.9±0.4×10 6 cells after laparotomy, 3.1±1.0 ×10 6 cells after amputation) when compared with controls (11.1±1.9× 10 6 cells and 10.3±1.3×10 6 cells) ( p <0.001 for laparotomy and p < 0.005 for amputation). The inhibitory effect of operative trauma was not abolished by bilateral adrenalectomy performed 12 days before laparotomy. In rats, surgical trauma induces a depression of remote inflammatory reactions. This phenomenon is not related to increased corticosterone levels.
Résumé
On a étudié le rôle de la chirurgie sur l'inflammation chez le rat R/A Wistar en utilisant le modèle d'oedème induit par la carragénine. L'augmentation de volume de la patte a été mesurée avec des unités arbitraires standardisées, 2, 4, et 6 heures après l'injection sous-cutanée de carragénine iota dans la région plantaire de la patte arrière droite. Ce test a montré qu'après laparotomie, ces valeurs se réduisaient à 5.0±0.4, 8.0 ±1.0, 13.7±1.9 par rapport à celles des animaux contrôles (anesthésiés simplement à l'ether): 6.2±0.5, 15.5±1.2, 23.7±0.6) (p < 0.001 à 4 et à 6 heures). Cette inhibition a duré au moins 24 heures et a été observée après amputation mais dans ces expériences la différence entre les animaux opérés et les animaux contrôles n'était pas significative. Les modifications de l'infiltration inflammatoire cellulaire ont été étudiées en implantant en sous-cutanée des éponges en polyuréthane, trempées dans la carragénine lambda chez des animaux contrôles et chez des rats soumis à une laparotomie ou une amputation. Le nombre de cellules d'éponge récupérée 5 heures après l'implantation était plus petit (2.9±0.4×106 cellules après laparotomie, 3.1±1.0×106 cellules après amputation), comparé à celui des contrôles (11.1±1.9×106 et 10.3±1.3×106 cellules) (p < 0.001 pour la laparotomie, p<0.005 après amputation). L'effet inhibiteur du traumatisme opératoire n'a pas été aboli par la surrénalectomie bilatérale effectuée 12 jours avant la laparotomie. Chez le rat, le traumatisme chirurgical induit une dépression des réactions inflammatoires à distance. Ce phénomène n'est pas en rapport avec une augmentation des taux d'hormones corticostéroÏdes.
Resumen
El efecto de la cirugía sobre la respuesta inflamatoria, primera lí'nea de defensa del cuerpo contra la invasión por microorganismos, fue estudiada en ratas Wistar R/A utilizando el modelo de edema inducido por carrageenina. El edema de la pata fue medido en unidades estandarizadas arbitrarias a las 2, 4, y 6 horas después de la inyección subcutánea de carrageenina iota en la región subplantar de la pata trasera derecha. El fenómeno apareció significativamente deprimido en ratas sometidas a laparotomía (5.0±0.4, 8.0±1.0, 13.7±1.9), en comparación con las ratas control que fueron simplemente anestesiadas con éter(6.2±0.5, 15.5±1.2, 23.7±0.6) (p < 0.001 a las 4 y alas 6 horas). Tal inhibición se mantuvo por lo menos por 24 horas y también fue observada después de amputación, aunque en estos experimentos no se encontró diferencia significativa entre los animales operados y lps controles. Las alteraciones del infiltrado inflamatorio celular fueron estudiadas utilizando esponjas impregnadas con carrageenina lambda e implantadas en el tejido subcutáneo en animales control y en ratas sometidas a laparotomía o a amputación. El nÚmero total de células recuperadas de estas esponjas 5 horas después de su implantación fue menor en las ratas operadas (2.9±0.4×106 células después de laparotomía, 3.1±1.0×106 células después de amputación) en comparacion con las ratas control (11.1±1.9×106 células y 10.3±1.3×106 células) (p < 0.001 para laparotomía yp< 0.005 para amputación. El efecto inhibitorio del trauma operatorio no fue abolido por la adrenalectomía bilateral realizada 12 días antes de la laparotomía. En las ratas el trauma quirÚrgico induce una depresión de las reacciones inflamatorias remotas. Este fenómeno no está relacionado con niveles aumentados de corticosterona.
