Penetration of ampicillin into urethral and cervical secretions after oral administration of bacampicillin

Summary

The study comprised 24 male and 24 female patients treated either for uncomplicated gonorrhoea or as a prophylaxis. A single dose of 800 mg of bacampicillin +1.0 g probenecid was administered orally to each patient. The concentrations of ampicillin in urethra and cervix were assayed by means of alginate swabs that were used as diffusion centres on agar diffusion plates. The inhibition zones were compared with a standard series of swabs containing known amounts of ampicillin. The amounts of secretions absorbed into the swabs were determined by weighing each swab before and after taking the specimen. The median ampicillin concentration in the male urethra one hour after the single dose of bacampicillin plus probenecid was 2.0 µg/ml of urethral secretion and increased to 3.2 µg/ml two hours after administration of the antibiotic. In the female urethra, the median concentrations of ampicillin were 1.5 µg/ml and 2.3 µg/ml of urethral secretion one and two hours, respectively, after the same dosage as above. The corresponding ampicillin concentrations in the cervical secretion were 1.6 µg/ml and 2.9 µg/ml, respectively.

Zusammenfassung

Die Untersuchung erfaßte 24 männliche und 24 weibliche Patienten, die entweder wegen unkomplizierter Gonorrhoe oder prophylaktisch behandelt wurden. Jedem Patienten wurde eine Dosis von 800 mg Bacampicillin und 1,0 g Probenecid verordnet. Die Konzentration von Ampicillin in Urethra und Cervix wurde mit Hilfe von Alginate Tupfern bestimmt, die als Diffusionszentren auf Agar-Diffusionsplatten benutzt wurden. Die Hemmzonen wurden mit einer Standardserie von Tupfern verglichen, die bekannte Dosen von Ampicillin enthielten. Die Menge Sekret, die in die Tupfer aufgenommen wurde, wurde durch Wiegen jedes Tupfers vor und nach Probenahme bestimmt. Die mittlere Ampicillinkonzentration in der männlichen Urethra eine Stunde nach der Einzeldosis Bacampicillin plus Probenecid betrug 2,0 µg/ml Urethra-Sekret und nahm zwei Stunden nach Antibiotikagabe auf 3,2 µg/ml zu. In der weiblichen Urethra betrugen die mittleren Ampicillinkonzentrationen 1,5 µg/ml und 2,3 µg/ml Urethral-Sekret eine bzw. zwei Stunden nach der oben genannten Dosis. Die entsprechenden Ampicillinkonzentrationen im Cervikal-Sekret waren jeweils 1,6 µg/ml und 2,9 µg/ml.
