Functional results after postirradiation rectal reconstruction

Abstract

The functional results obtained in 26 patients operated on for postirradiation rectovaginal fistula, stricture, or combination of fistula and stricture are reported in detail. Emphasis is placed on the degree of continence obtained. The operation consists of use of proximal nonirradiated colon to repair the damaged area without doing a rectal resection. It has been found that nonirradiated colon with a good blood supply can be expected to heal to irradiated colon or rectum and complete resection of the entire irradiated area is not necessary. The results of this operation have been excellent in 11, satisfactory in 8, and poor in 2; the procedure failed in 2 patients. One patient died as a consequence of the operation and 2 patients died of other causes before having closure of a temporary colostomy. It was found that the presence of radiation damage to the small intestine could influence the quality of the end result.

Résumé

Les résultats fonctionnels obtenu chez 26 malades qui ont été opérés pour des fistules recto-vaginales, des sténoses ou pour l'association de fistules et de sténoses secondaires à l'irradiation sont relatés en détail, particulièrement en ce qui concerne le degré de la continence. L'opération consiste en l'emploi du colon proximal indemne de toute lésion pour procéder à la cure, et ce, sans sacrifier le rectum. Les auteurs ont pu constater que le colon dès lors qu'il possédait une bonne vascularisation pouvait Être anastomosé au colon ou au rectum qui avait été irradié sans qu'il soit nécessaire de procéder à l'exérèse complète de toute la région irradiée. Les résultats de l'intervention ont été excellents dans 11 cas, satisfaisants dans 8 cas, médiocre dans 2 cas et mauvais dans 2 cas. Un malade est décédé du fait de l'intervention, deux sont morts de causes indépendantes de l'opération avant la suppression de la colostomie. Il a été également constaté que les lésions radiques de l'intestin grÊle pouvaient retentir sur la qualité du résultat final.

Resumen

Se informa en detalle el resultado funcional en 26 pacientes operados por fístula rectovaginal, estrechez o combinación de fístula y estrechez, resultantes de irradiación. Se hace énfasis en el grado de continencia que se puede lograr. La operación se fundamenta en la utilización del segmento proximal no irradiado del colon para reparar el área lesionada, sin resección del recto. Se ha demonstrado que el colon con buena vascularidad puede ser unido al colon o al recto irradiados con buena cicatrización y que no es necesario realizar la resección completa de la totalidad del área irradiada. Un paciente murió como consecuencia de la operación y otros dos pacientes murieron por otras causas antes de que se pudiera realizar el cierre de la colostomía temporal. Se encontró que la presencia de lesión irradiación en el intestino delgado puede influenciar la calidad del resultado final.
