Abstract
Four hundred thirty-nine patients with autonomously functioning thyroid tissue (AFTT), 26% of all patients with AFTT diagnosed at the University of Marburg between 1970 and 1982, underwent thyroidectomy. One hundred ninety (43.3%) of these patients with AFTT were operated on for clinical hyperthyroidism, compared to only 62 patients with Graves' disease during this same time period. The remaining 249 patients with AFTT underwent surgery because of nonsuppressible uptake of technetium (TcTU) (3.5% or greater) by the thyroid.
AFTT was 5 times more common in women than men. Hyperthyroidism was more common in older patients (p < 0.001) and in patients with numerous autonomous nodules. Many patients had no obvious signs of hyperthyroidism, but 42.4% had cardiac disorders. A preceding history of goiter (mean 15.3 years) was present in patients with AFTT and hyperthyroidism. Serum T 3 and T 4 levels were elevated in 88.5% and 74.6% of AFTT patients with clinical hyperthyroidism, while in clinically euthyroid AFTT patients, T 3 and T 4 values were in the upper normal range, i.e., 53.7% and 15.2%, respectively.
Hyperthyroid patients were prepared for operation with antithyroid drugs. Additionally, propranolol was used in the perioperative period. Patients were operated on selectively after 1979, either by enucleation or unilateral thyroidectomy for unifocal AFTT, and subtotal thyroidectomy for bilateral disease. The results of treatment were most satisfactory and only 11 patients with AFTT (2.5%) developed recurrent hyperthyroidism. Surgical treatment of patients with hyperthyroidism and AFTT is a safe and effective therapeutic modality.
Résumé
De 1970 à 1980, quatre cent trente-neuf malades (439) présentant du tissu thyroïdien fonctionnant de façon autonome, c'est à dire 26% des malades de ce type, ont subi une thyroïdectomie. Cent quatrevingt-dix (190) soit 43% furent opérés pour hyperthyroïdisme clinique alors que seulement soixante (60) malades atteints de maladie de Basedow subissaient une intervention pendant cette même période. Les deux cent quarante-neuf autre (249) furent opérés en raison de la persistance d'un taux de fixation du technetium (TcTU) par le tissu thyroïdien égal ou supérieur à 3.5%. Ce type de lésion est cinq fois plus fréquent chez l'homme que chez la femme, l'hyperthyroïdisme étant plus fréquent chez les sujets âgés (p<0.001) et les porteurs de nodules autonomes multiples. De nombreux patients n'accusaient pas de signes d'hyperthyroïdisme mais 42.4% souffraient de troubles cardiaques. Des antécédents de goitre (durée moyenne de 15.3 ans) existaient chez les sujets hyperthyroïdiens. Les taux sériques de T3 et de T4 étaient de 88.5% et 74.6%. Chez les malades présentant un hyperthyroïdisme clinique alors que les taux sériques de T3 et T4 étaient de 53.7% et 15% chez les sujets euthyroïdiens. Les malades hyperthyroïdiens furent préparés par des drogues antithyroïdiennes avant l'intervention et parfois le propanolol fut employé avant et après l'intervention. Après 1979 les opérés subirent soit une énucléation, soit une lobectomie en cas de lésion unifocale ou une thyroïdectomie subtotale en cas de lésions bilatérales. Les résultats opératoires furent très satisfaisant, onze opérés (11) soit 2.5% seulement présentèrent une récidive. En conclusion, le traitement chirurgical de ces lésions s'accompagnant d'hyperthyroïdisme est dénué de danger et efficace.
Resumen
La aparición de tejido tiroidea con funcionamiento autónomo (TTFA) ocurre con mayor frecuencia en áreas de bocio endémico y deficiencia de yodo. Aún cuando la mayoría de los pacientes con TTFA en áreas de bocio endémico son eutiroideos, algunos se hacen hipertiroideos. Entre el 50 y el 60% de los pacientes de edad avanzada con nódulos grandes hiperfuncionantes o con nódulos autónomos múltiples desarrollan o presentan hipertiroidismo. La incidencia de hipertiroidismo también se ve incrementada en áreas deficientes en yodo cuando el contenido dietario de yodo es aumentado (hipertiroidismo de Jod-Basedow).
Cuatrocientos treinta y nueve pacientes con TTFA, que representan el 26% de todos los pacientes con TTFA diagnosticados en la Universidad de Marburgo entre 1970 y 1982, fueron sometidos a tiroidectomía. Ciento noventa (43.3%) de estos pacientes con TTFA fueron intervenidos por hipertiroidismo clínico, en comparación con sólo 62 pacientes con enfermedad de Graves operados en el mismo período. Los 249 pacientes restantes con TTFA fueron intervenidos porque exhibían captación tiroidea no supresible de tecnecio (TcTU) de 3.5% o superior.
El TTFA apareció con una frecuencia cinco veces mayor en las mujeres que en los hombres. El hipertiroidismo ocurrió con mayor frecuencia en pacientes de edad avanzada (p<0.001) y en pacientes con múltiples nódulos autónomos. Muchos pacientes no presentaban signos obvios de hipertiroidismo, pero el 42.4% exhibió alteraciones cardiacas. Historia antecedente de bocio (promedio de 15.3 años) fué registrada en los pacientes con TTFA e hipertiroidismo. Los nivelés séricos de T3 y T4 aparecieron elevados en los pacientes con TTFA con hipertiroidismo clínico (T3 en el 88.5%, T4 en el 74.6%, T3+T4 en el 63.1%), en tanto que los valores en pacientes con TTFA y clínicamente eutiroideos aparecieron en el nivel normal superior (T3, 53.7%; T4, 15.2%).
Los pacientes hipertiroideos fueron preparados para la operación con drogas antitiroideas; además, se utilizó propranol en el período perioperatorio. La extensión de la resección ha cambiado en los años recientes (1972–1982). Operaciones unilaterales se realizan ahora con mayor frecuencia para bocios unilaterales; previamente tales pacientes eran tratados por medio de la resección bilateral. O sea, que en la época reciente se viene realizando una operación selectiva, consistente en enucleación o tiroidectomía subtotal para enfermedad bilateral. Los resultados del tratamiento han sido muy satisfactorios y sólo 11 pacientes con TTFA (2.5%) han desarrollado hipertiroidismo recurrente. El tratamiento quirúrgico de pacientes con hipertiroidismo y TTFA es una modalidad terapéutica segura y efectiva y nuestra experiencia, así como la de otros, indica que virtualmente todo paciente con nódulos de funcionamiento autónomo de 3.5 cm o mayores, o con una captación de TcTU de m'as de 3.5% después de terapia supresiva deben ser sometidos a tiroidectomía. El yodo radioactivo está indicado solamente en aquellos pacientes que representan un alto riesgo operatorio.
