The Pennsylvania State University paracorporeal ventricular assist pump: Optimal methods of use

Abstract

A ventricular assist device (VAD) has been developed for clinical use at The Pennsylvania State University that can be inserted with standard surgical technique, is capable of completely supporting either the systemic or pulmonary circulation, and does not require anticoagulation. Since 1976, this device has been used in 29 patients following cardiac surgical procedures who were not able to be weaned from cardiopulmonary bypass (CPB) with conventional techniques. From 1976 through 1979, we used ventricular assist pumping in 14 patients. Four of these patients were weaned from the assist pump and 2 were discharged from the hospital, yielding a 14% survival rate. From 1980 to the present, 16 patients required use of the assist pump from which 9 patients were weaned and 7 patients were discharged yielding a 44% survival rate. During the 8 years of clinical experience with this device, a program designed to optimize results has evolved with respect to patient selection, insertion technique, and postoperative care that has resulted in significant improvement in patient survival.

Résumé

Une pompe d'assistance ventriculaire qui a été mise au point pour application clinique à l'Université de l'Etat de Pennsylvanie peut être mise en place en utilisant les techniques chirurgicales classiques et peut suppléer complètement soit la circulation systémique, soit la circulation pulmonaire, sans nécessiter d'anticoagulation. Depuis 1976, cette pompe a été utilisée chez 29 malades après des interventions de chirurgie cardiaque, malades qui ne pouvaient être sevrés de la circulation extracorporelle en utilisant les moyens conventionnels. De 1976 à 1979, 14 malades ont été placés sous assistance ventriculaire: 4 de ces malades ont pu être sevrés de la pompe d'assistance et 2 sont sortis de l'hôpital, montrant un taux de survie de 14%. De 1980 à aujourd'hui, 16 malades ont nécessité une pompe d'assistance: 9 d'entre eux ont pu être sevrés et 7 patients sont sortis de l'hôpital montrant cette fois un taux de survie à 44%. Pendant les 8 années d'expérimentation clinique de cette pompe, les critères pour optimiser les résultats ont évolué en tenant compte de la sélection des malades, de la technique d'insertion de la pompe et de la réanimation postopératoire facteurs qui peuvent amener une amélioration significative du taux de survie des malades.

Resumen

Un sistema de asistencia mecánica ventricular (AMV) para uso clínico ha sido desarrollado por la Pennsylvania State University, el cual puede ser insertado mediante técnica quirúrgica estandar, es capaz de un soporte completo de la circulación sistémica o de la circulación pulmonar, y no requiere anticoagulación. A partir de 1976 el sistema ha sido utilizado en 29 pacientes quienes, después de procedimientos quirúrgicos cardiacos, no pudieron ser desahijados de la perfusión cardiopulmonar extracorpórea mediante las técnicas convencionales. A partir de 1976 y hasta el final de 1979 utilizamos el bombeo de AMV en 14 pacientes. Cuatro de ellos pudieron ser desahijados de la bomba de AMV y dos fueron dados de alta del hospital, lo cual significa una tasa de supervivencia de 14%. Desde 1980 y hasta el presente, 16 pacientes han requerido el uso de la bomba de AMV, de la cual nueve pacientes pudieron ser desahijados y siete pacientes fueron dados de alta, para una tasa de supervivencia de 44%. En el curso de los ocho años de experiencia clínica con este sistema se ha desarrollado un programa destinado a optimizar los resultados, relativo a la selección de los pacientes y técnica de inserción y cuidado postoperatorio, el cual ha resultado en un incremento significativo de la tasa de supervivencia.
