Progress in the care of the victim of multiple injuries

Abstract

Progress over the past 20 years in the care of victims of multiple injuries has been largely technical. These technological advances, which have made it possible to study both the structural and functional effects of injury on all organ systems, have also brought difficulties—increased septicemia from invasive measures, changes that result from the treatment that may themselves prove fatal, and life prolonged beyond any meaningful existence.

Progress, however, has also been made in therapy, in the understanding of the pathophysiology, and in the training and equipping of emergency room staff. It is these individuals who have played a large part in the progress in this field.

Résumé

Les progrès enregistrés dans le traitement des poly-traumatisés au cours des 20 dernières années sont essentiellement d'ordre technique. Ces améliorations techniques ont permis d'étudier les effets du traumatisme sur la structure et la fonction des différents organes au prix parfois 1) de l'augmentation du taux de l'infection en raison de l'emploi de méthodes invasives, 2) de décés dûs a l'introduction de nouvelles méthodes thérapeutiques, 3) du maintien alusif de la vie lorsque les lésions sont irréversibles. En dépit de ces faits négatifs des progrès considérables ont été accomplis dans la connaissance de la physiopathologie des traumatismes multiples ce qui a conduit à l'amélioration de l'équipement avec l'entraînement des services d'urgence voués au traitement des polytraumatisés.
