Abstract
Even when thyroidectomy with lymphadenectomy for thyroid carcinoma is macroscopic-ally radical, postoperative scintigraphy sometimes reveals iodine uptake in residual or ectopic thyroid tissue or lymph nodes, necessitating radioiodine ablation. In order to detect such tissue during the operation, a technique for intraoperative scintigraphy was devised. It was tested in 20 patients undergoing total thyroidectomy for highly differentiated thyroid carcinoma. The patients received 20 mBq125I intravenously 24 hours before the operation. After removal of all macroscopically visible thyroid tissue and metastatic growth, the operative field was examined with a cadmium-telluride detector placed in a sterile tube of stainless steel. By using this technique, iodine uptake was found in 16 of the 20 patients. All of this tissue was removed in 11 patients, leading to a negative postoperative scintiscan in 9 patients. In 2 patients, the perioperative scanning identified minimal residual uptake in a minute remnant of thyroid tissue adjoining the nerve entrance and not requiring postoperative radioiodine ablation. Scintigraphy was repeated in all patients 6 weeks after thyroidectomy. Recordings over different-sized tissue fragments showed the detector to have a very high sensitivity, with capacity to detect tissue fragments weighing less than 2 mg. The results indicate that intraoperative scintigraphy, according to this technique, increases the possibilities to perform a complete removal of thyroid and tumor tissue and significantly reduces the need of postoperative radioiodine ablation.
Résumé
Alors même que la thyroïdectomie avec lymphadectomie pour cancer de la thyroïde semble macroscopiquement complète, la scintigraphie postopératoire met parfois en évidence des foyers restants au niveau du tissu thyroïdien ectopique ou des ganglions; foyers qui nécessitent d'être traités par l'iode radioactif. Dans le but de les déceler au cours de l'intervention une technique de scintigraphie peropératoire a été conçue. Elle a été testée chez 20 sujets qui ont subi une thyroïdectomie totale pour cancer thyroïdien hautement différencié. Ils ont reçu 20 mBq125I par voie intraveineuse 24 heures avant l'intervention. Après ablation complète en apparence de la totalité du parenchyme thyroïdien pathologique et des ganglions le champ opératoire a été exploré par un détecteur (tellurate de cadmium placé dans un tube stérile d'acier inoxydable). Dans 16 cas des foyers restants furent découverts. Onze fois ces foyers furent éradiqués la scintigraphie confirmant la totalité de l'exérèse chez 9 d'entre eux. Deux fois la scintigraphie peropératoire décela la présence d'un minuscule foyer actif au voisinage du nerf, ce foyer ne nécessitant pas ultérieurement l'emploi d'iode radioactif. La scintigraphie fut repétée chez tous les opérés 6 mois après la thyroïdectomie. Les enregistrements au niveau des fragments de tissu thyroïdien de taille diverse ont montré que ce détecteur avait une grande sensibilité et était capable de déceler des moignons insulaires dont le poids était inférieur à 2 mg. Les résultats de la scintigraphie peropératoire démontrent que cette technique permet d'accomplir l'exérèse complète de la tumeur thyroïdienne et aussi de réduire l'irradiation postopératoire par l'iode radio-isotopique.
Resumen
Aún cuando la tiroidectomía con lifadenectomía realizada por carcinoma tiroideo aparece macroscópicamente como radical, la centelleografía postoperatoria en ocasiones revela captación en tejido tiroideo residual o ectópico, o en ganglios linfáticos, lo cual hace necesaria la erradicación mediante yodo radioactivo. Con el objeto de identificar tal tipo de tejido en el curso de la operación, se ha disenado una técnica de centelleografía intraoperatoria. Tal técnica ha sido probada en 20 pacientes sometidos a tiroidectomía total por carcinoma altamente diferenciado. Los pacientes recibieron 20 mBq125I por vía intravenosa 24 horas antes de la operación. Después de la resección de todo el tejido tiroideo y metastásico macroscópicamente visible, el campo operatorio fue examinado con un detector de telúrido de cadmio colocado dentro de un tubo estéril de acero inoxidable. Captación de yodo radioactivo fue detectada con esta técnica en 16 de 20 pacientes. Todo el tejido fue resecado en 11 casos, lo cual resultó en centelleograffa negativa postoperatoria en 9 pacientes. En 2 pacientes la centelleografía perioperatoria identificó captación residual minima en un remanente minúsculo de tejido tiroideo adyacente a la entrada del nervio recurrente laríngeo al cartílago cricolaríngeo, lo cual no hizo necesaria la erradicación con yodo radioactivo. La centelleografía fue repetida en la totalidad de los pacientes a las 6 semanas después de la tiroidectomia. La medición efectuada sobre fragmentos de tejido tiroideo de diversos tamaños demostró que el detector posée un alto grado de sensibilidad, con capacidad para detectar fragmentos de tejido con peso inferior a 2 mg. Los resultados indican que la centelleografía intraoperatoria realizada mediante esta téenica incrementa las posibilidades de realizar una resección completa de un tumor tiroideo y reduce en forma significativa la necesidad de realizar la erradicación postoperatoria con yodo radioactivo.
