Bronchobiliary fistula due to stones in the biliary tree: Report of two cases

Abstract

Two patients with bronchobiliary fistula are reported. Each had a history of biliary disease with operations done prior to the onset of bilioptysis. Obstruction of the biliary tree by stones was the etiology in both cases. The diagnosis was made by detection of bile in the sputum. The definitive treatment consisted of removal of the obstructing stones and excision of the fistula. If emergency surgery is required because of sepsis, external drainage of the abscess and T-tube drainage should be performed first, followed by definitive procedure after recovery.

Résumé

Deux cas de fistules bilio-bronchiques sont rapportées dans cet article. Dans les deux cas existait une histoire d'affection biliaire ayant donné lieu à une intervention chirurgicale. L'obstruction de l'arbre biliaire par des calculs était à l'origine de ces deux cas. Le diagnostic avait été porté sur la constatation de présence de bile dans les crachats.

Le traitement radical consiste en l'ablation des calculs et en l'exérèse du trajet fistuleux. Si l'intervention apparait urgente en raison de l'existence d'un état septique, le drainage externe de l'abcès et la mise en place d'un tube en T dans la voie biliaire principale s'imposent avant d'avoir recours au traitement définitif lorsque l'état général du malade est devenu satisfaisant.
