Selective pressure of antistaphylococcal chemotherapeutics in favour of pseudomonas aeruginosa in cystic fibrosis

Summary

Pseudomonas aeruginosa is insensitive against the majority of oral chemotherapeutics active against staphylococci. During application of such compounds in patients colonized byP. aeruginosa the concentration ofP. aeruginosa increased in the majority of treatment episodes by factors between 2.5 and 50000. So, from a microbiological point of view the application of purely antistaphylococcal drugs should not be used prophylactically against staphylococci.

Zusammenfassung

Orale Chemotherapeutika mit Antistaphylokokkenaktivität sind zumeist unwirksam gegenüberPseudomonas aeruginosa. In der Mehrzahl der Sputa von Mukoviszidosepatienten mitP. aeruginosa-Dauerkolonisation stieg die Konzentration vonP. aeruginosa bei Gabe von Antistaphylokokken-Chemotherapeutika ohne Antipseudomonasaktivität an (Faktor 2,5 bis 50000). Der Einsatz von Antistaphylokokken-Antibiotika ohneP. aeruginosa-Wirkung bei fehlendem Nachweis von Staphylokokken erscheint aus mikrobiologischer Sicht bedenklich.
