Zusammenfassung
Sisomicin wurde von mehr als 80 Ärzten in den USA, in Deutschland und 12 anderen europäischen, südamerikanischen und afrikanischen Ländern klinisch geprüft. Die Prüfungen wurden gemeinsam von der Medical Research Division der Schering-Plough Corporation, Bloomfield, USA, und dem Ressort Medizin der Bayer AG in Deutschland betreut und ausgewertet. 1370 Patienten erhielten 1473 Sisomicin-Behandlungsserien zur Therapie von 1583 Infektionen, hauptsächlich des Urogenitaltraktes, der Atemwege, des Gastrointestinaltraktes, der Weichteile oder Knochen und septischer Infektionen. 84 % der Infektionen konnten günstig beeinflußt werden: In 64 % wurde Heilung und in 20 % deutliche oder mäßige Besserung erzielt. In weiteren 3 % der Infektionen trat eine leichte Besserung ein, 13 % blieben unbeeinflußt. Von den 1728 vor Therapiebeginn isolierten Erregern konnten 77 % eliminiert und 3 % stark vermindert werden, 20 % persistierten. Die Verträglichkeit wurde anhand sorgfältiger klinischer Beobachtungen und umfangreicher Laboruntersuchungen beurteilt. Eine Beeinträchtigung der Nierenfunktion wurde in 1,9 % der Behandlungen als wahrscheinlich behandlungsbedingt angesehen; bei weiteren 5,5 % bestand ein möglicher Zusammenhang mit der Sisomicin-Therapie, und in 1,5 % war der Zusammenhang zweifelhaft. Die Veränderungen der Nierenfunktion erwiesen sich in der Mehrzahl der Fälle, in denen ausreichende Kontrollen möglich waren, als reversibel. In 0,6 % der Behandlungen ließen sich Anzeichen otovestibulärer Funktionsstörungen in wahrscheinlichem Zusammenhang mit der Sisomicin-Therapie beobachten, in 1,1 % standen solche Symptome in möglichem und in 0,4 % in zweifelhaftem Zusammenhang mit dieser Therapie. Andere Begleiterscheinungen, wie Anstieg der alkalischen Phosphatase, der SGPT und/oder SGOT, allergische Hautreaktionen, Pruritus, Erbrechen oder Kopfschmerzen, traten sehr selten auf und waren niemals schwerwiegender Natur. Die klinische Prüfung hat ergeben, daß Sisomicin ein hochwirksames Aminoglykosid-Antibiotikum ist und daß seine Verträglichkeit mit der anderer Antibiotika dieser Stoffklasse vergleichbar ist.
Summary
Clinical research on sisomicin was conducted by over 80 investigators from the United States, Germany and 12 other countries in Europe, South America, and Africa. The trials were monitored and evaluated jointly by the medical research divisions of Schering-Plough Corporation, Bloomfield, USA, and Bayer AG, Federal Republic of Germany. 1370 patients received 1473 courses of treatment with sisomicin for 1583 infections, primarily of the genitourinary, respiratory or gastrointestinal tracts, soft tissue or bone, and for septicaemia. Favorable responses were reported in 84 % of the infections. In 64 % of the infections complete resolution was achieved and in 20 % a marked or moderate improvement. Slight improvement was noted in 3 %, and failure to respond to treatment was reported in 13 %. Of the 1728 causative organisms isolated 77 % were eliminated, 3 % were markedly reduced, and 20 % persisted. The tolerance of sisomicin was evaluated by means of careful clinical observation and a comprehensive battery of laboratory tests. Impairment of renal function that could probably be associated with administration of the drug was reported in 1.9 % of the courses. In 5.5 % impairment of renal function was possibly related to sisomicin therapy and in an additional 1.5 % the relationship was doubtful. In most of the patients in whom follow-up results were available impairment of renal function was reversible. Signs of otovestibular dysfunction were considered to be probably related to sisomicin administration in 0.6 % of the courses, possibly related in 1.1 %, and doubtfully related in 0.4 %. Other adverse reactions were infrequent and mild in nature and included increased levels of alkaline phosphatase, SGPT and/or SGOT, allergic skin reaction, pruritus, vomiting, and headache. These clinical investigations demonstrate that sisomicin is a highly effective aminoglycoside antibiotic and that its tolerance is comparable to that of other antibiotics of its class.
