Zusammenfassung
Es wird über Ergebnisse der Sisomicin-Behandlung an 33 Patienten mit 49 Infektlokalisationen berichtet. Mit Sisomicin als Monotherapie wurde in 77 % und in Kombination mit anderen Antibiotika in 72 % Heilung erzielt. Die Heilungsquote insgesamt betrug 76 %, Besserungen wurden in 18 % der Fälle gesehen. Nur bei drei der 49 Infektionen war Sisomicin erfolglos. Die infektionsauslösenden Erreger wurden — soweit Kontrollen möglich waren — ausnahmslos eliminiert. Drei Superinfektionen und eine Reinfektion mit einem neuen Keim wurden zwei Wochen nach Abschluß der Therapie beobachtet. Die lokale Verträglichkeit war gut. Störungen der otovestibulären Funktion traten nicht auf. In zwei Fällen, die extrem lange (23 bzw. 35 Tage) mit Sisomicin und zeitweise in Kombination mit Cephalosporin-Derivaten behandelt worden waren, wurde eine vorübergehende, leichte Einschränkung der Nierenfunktion festgestellt. Die Empfindlichkeit von 198 klinischen Isolaten gegen Sisomicin, Gentamicin und Tobramycin wurde im Reihenverdünnungstest geprüft. Tobramycin war gegen Pseudomonas aeruginosa am wirksamsten, gegenüber allen anderen Keimen (Staphylokokken, Streptokokken, Klebsiella spp., E.coli, Proteus mirabilis, indol-positive Proteus spp. und Enterokokken) war Sisomicin die aktivste Substanz.
Summary
The present paper reports on the results of sisomicin treatment in 33 patients in whom a total of 49 infections had been diagnosed. Cure was achieved in 77 % of cases treated with sisomicin alone, and in 72 % of cases treated with sisomicin in combination with other antibiotics. The total cure rate was 76 %; improvement was recorded in 18 % of cases. Only three of the 49 infections did not respond to sisomicin. The causative organisms were eliminated in all cases where follow-up was possible. Three superinfections and one reinfection with a new pathogen two weeks after termination of therapy were diagnosed. Sisomicin was well tolerated locally, disturbances of the function of the eight cranial nerve did not occur. Two patients who had been undergoing protracted treatment (23 and 35 days resp.) with sisomicin, combined for a certain time with cephalosporin derivatives, presented with slight transient impairment of renal function. The sensitivity of 198 isolates to sisomicin, gentamicin and tobramycin was examined using the tube dilution test. Tobramycin proved to be the most effective agent against Pseudomonas aeruginosa, whereas sisomicin was the most active substance against all other microorganisms (staphylococci, streptococci, Klebsiella spp., E. coli, Proteus mirabilis, indole-positive Proteus spp., and enterococci).
