Summary
Sisomicin, a newly developed aminoglycoside, was administered parenterally to 119 surgical patients with infections of the peritoneum and various wounds. The average dose was 1 mg/kg body weight/8 hours, as adjusted to serum creatinine levels. All patients were monitored for toxicity in the inner ear, kidneys, bone marrow, and liver. Final success of antibiotic therapy appeared to be primarily dependent upon the eventual establishment of surgical drainage, yet antibiotic therapy was exceedingly important up until then in maintainance of life and in pre-operative preparation. Bacteriologic analysis demonstrated a predominance of gram-negative rods, with susceptibility being greater to sisomicin than to other antibiotics as tested by disc and tube dilution methods. Specific drug toxicity was not encountered, except for a possible adverse contribution to pre-existing ischemic renal damage in two of the patients.
Zusammenfassung
Sisomicin, ein neues Aminoglykosid, wurde parenteral 119 chirurgischen Patienten verabreicht, die an Infektionen des Peritoneums und verschiedener Wunden litten. Die Durchschnittsdosis betrug 1 mg/kg alle 8 Stunden und wurde den Serum Kreatinin-Spiegeln angepaßt. Alle Patienten wurden auf die Verträglichkeit für das Innenohr, die Nieren, das Knochenmark und die Leber sorgfältig untersucht. Der Erfolg der Antibiotika-Therapie schien abhängig zu sein von der Anlage einer chirurgischen Drainage, wenngleich die antibiotische Therapie bis zu diesem Zeitpunkt ungemein wichtig war, um dem Patienten das Leben zu erhalten und seine Operation vorzubereiten. Bakteriologische Untersuchungen zeigten ein Überwiegen gramnegativer Stämme, deren Empfindlichkeit größer für Sisomicin als für andere Antibiotika war, getestet im Diffusions- und Reihenverdünnungstest. Eine spezifische auf Sisomicin zu beziehende Toxizität wurde nicht gefunden, mit Ausnahme zweier Patienten, bei denen ein vorbestehender ischämischer Nierenschaden möglicherweise negativ beeinflußt worden ist.
