Summary
Sisomicin, a recent addition to the aminoglycoside group of antibiotics, is structurally related to the gentamicin complex, but is a monosubstance closely resembling gentamicin C1a. The present investigation was designed to study the antibacterial activity, pharmacokinetics, and the clinical effectiveness and tolerance of the antibiotic. The sensitivity tests on a fairly large number of clinical isolates did not reveal any major differences between the activities of sisomicin and gentamicin. The fact that a slightly larger number of Pseudomonas strains were sensitive to less than 1 µg/ml of sisomicin, than to the same concentration of gentamicin, might be clinically important. The pharmacokinetics of sisomicin were found to be similar to those of other aminoglycosides and indicate satisfactory distribution in tissues and infection sites. The assessment of therapeutic effects was complicated by several factors. The number of patients treated was small (25) but the study included a large proportion of severe infections, among others 11 cases of septicemia. Since in most cases the etiology of the infection was not established at the onset of therapy, a penicillin-type antibiotic was administered concomitantly to 18 patients. Of the seven patients given sisomicin alone, five were cured, and improvement was achieved in one. In one patient with pneumonia of unknown etiology sisomicin had no beneficial effect. Of the 18 patients given sisomicin together with benzyl penicillin or ampicillin, 12 were cured, one markedly improved, whereas failures were recorded in three patients. In two cases the clinical result was indeterminate. Treatment with sisomicin was generally well tolerated. Sisomicin appears to be a favorable alternative to gentamicin.

Sisomicin, eine neue Substanz in der Gruppe der Aminoglykosid-Antibiotika, ist strukturell mit dem Gentamicin-Komplex verwandt, besteht jedoch nur aus einer Komponente, die dem Gentamicin C1a ähnlich ist. Diese Studie wurde durchgeführt, um die antibakterielle Wirkung, die Pharmakokinetik, die klinische Wirksamkeit und die Verträglichkeit von Sisomicin zu prüfen. Die Empfindlichkeitsprüfung verhältnismäßig vieler klinischer Isolate zeigte keine größeren Unterschiede zwischen der Aktivität von Sisomicin und der von Gentamicin. Die Tatsache, daß eine etwas größere Anzahl von Pseudomonas-Stämmen empfindlicher für Sisomicin als für Gentamicin waren bei Konzentrationen von weniger als 1 µg/ml, mag klinisch wichtig sein. Die Pharmakokinetik von Sisomicin ist ähnlich der der anderen Aminoglykoside und weist auf eine ausreichende Verteilung im Gewebe und am Ort der Infektion hin.
Die Bewertung des therapeutischen Effekts wurde durch viele Faktoren erschwert. Die Patientenzahl war gering (25). Andererseits umfaßte die Studie eine Vielzahl von schweren Infektionen, u. a. 11 Fälle mit Septikämien. Da in den meisten Fällen die Ursache der Infektion zu Beginn der Behandlung nicht festgestellt werden konnte, wurde gleichzeitig ein Antibiotikum vom Penicillin-Typ in 18 Fällen verabreicht. Von 7 Patienten, die nur Sisomicin erhielten, wurden 5 geheilt und 1 Patient zeigte eine Besserung. Bei einem weiteren Patienten mit einer Pneumonie unbekannter Genese sprach Sisomicin nicht an. Von 18 Patienten, denen Sisomicin in Kombination mit Benzylpenicillin oder Ampicillin verabreicht wurde, trat bei 12 Patienten eine Heilung und bei 1 eine deutliche Besserung ein. In 3 Fällen war ein Versagen zu verzeichnen. In 2 Fällen konnte über das klinische Ergebnis keine Aussage gemacht werden. Im allgemeinen wurde die Behandlung mit Sisomicin gut vertragen. Sisomicin scheint eine empfehlenswerte Alternative zu Gentamicin zu sein.
