Zusammenfassung
Wirksamkeit und Verträglichkeit von Sisomicin wurden an 40 Kranken mit 44 vorwiegend schweren Infektionen geprüft. Indikationen waren bronchopulmonale Infektionen (15), Harnwegsinfektionen (19), Weichteilinfektionen bei Ulcus cruris (4), Gallenwegsinfektionen (3), Sepsis (2) und Peritonitis (1). 65 % dieser Infektionen konnten geheilt, 35 % deutlich bis mäßig gebessert werden. Versager wurden nicht beobachtet. Dieses ausgezeichnete klinische Ergebnis fand seine Bestätigung in den bakteriologischen Befunden: 88 % der 51 infektionsverursachenden, vorwiegend gramnegativen Erreger konnten eliminiert und 12 % deutlich reduziert werden. Bei 4 der 44 Infektionen kam es nach Elimination der ursprünglichen Erreger zu einer Superinfektion. Die klinischen und bakteriologischen Ergebnisse sprechen dafür, daß Mischinfektionen von Staphylokokken mit E. coli, Klebsiella sp., Enterobacter sp. und Pseudomonas aerug. eine überzeugende Indikation für den Einsatz dieses Antibiotikums darstellen. Sisomicin wurde lokal und allgemein gut vertragen, nur bei einem Patienten kam es zu einer leichten Erhöhung des Serum-Kreatinins und des Blutharnstoffes in zweifelhaftem Zusammenhang mit der Therapie. Bei einem anderen Patienten kam es zu einer allergischen Hautreaktion. Otovestibuläre Funktionsstörungen traten nicht auf. Bei sorgfältiger Dosierung unter Berücksichtigung von Körpergewicht, Lebensalter und Nierenfunktion ist nach unseren Erfahrungen nur eine geringe Nebenwirkungsquote zu erwarten.
Summary
The efficacy and tolerance of sisomicin was tested in 40 patients with 44 infections, most of which were serious. Indications for treatment were bronchopulmonary infections (15), urinary tract infections (19), soft tissue infections accompanying stasis ulcers (4), infections of the biliary tract (3), septicemia (2), and peritonitis (1). Resolution was achieved in 65 % of the infections; 35 % showed a marked or moderate improvement. There were no failures. These excellent clinical results were confirmed by the microbiological findings: 88% of the 51 predominantly gramnegative pathogens were eliminated and 12 % were markedly reduced in number. In 4 of the 44 infections superinfection with a new pathogen occurred after elimination of the original pathogens. The clinical and bacteriological results show that mixed infections of staphylococci with E. coli, Klebsiella sp., Enterobacter sp. and Pseudomonas aerug. are a strong indication for the use of this antibiotic. Sisomicin was well tolerated locally and systemically. In one case only a slight increase of serum creatinine and blood urea was observed, the relation of which to the treatment was doubtful. Another patient showed an allergic skin reaction. No signs of otovestibular dysfunction were observed. According to our experience a low occurrence of side-effects can be expected if the dosage of sisomicin is carefully calculated on the basis of body weight, age, and renal function of the patients.
