Zusammenfassung
Das Aminoglykosid-Antibiotikum Sisomicin wurde in einer Reihe von Untersuchungsserien auf seine antibakterielle Aktivität geprüft. Mit dem Agar-Diffusionstest erfolgte die Untersuchung an 1280 Bakterienstämmen verschiedener Spezies aus klinischen Untersuchungsproben. Von 200 Stämmen wurde die Bestimmung der minimalen Hemm- bzw. Bakterizidie-Konzentration (MHK bzw. MBK) (Vergleichssubstanz: Tobramycin) vorgenommen. Die während einer klinischen Studie vor der Sisomicin-Behandlung gewonnenen Urin-Stämme wurden parallel mittels Diffusions- und Verdünnungstest untersucht. Durch die O2-Verbrauchsmessung unter Sisomicin-Einfluß bei je fünf Stämmen von Staph. aureus, E. coli, Proteus mirabilis und Pseudomonas aeruginosa fand eine quantitative und zeitliche Analyse der Sisomicin-Wirkung statt. Ergänzungsuntersuchungen befaßten sich mit dem O2-Verbrauch von sechs Pseudomonas-Stämmen bei Kanamycin-, Gentamicin-, Tobramycin-, Amikacin- und Sisomicin-Zugabe. Am Infektions- und Therapiemodell der akuten E.-coli- bzw. Proteusmirabilis-Pyelonephritis der Ratte geschah die Erfassung der keimreduzierenden Wirkung von Kanamycin, Gentamicin, Tobramycin und Sisomicin. Generell zeigte Sisomicin hohe antibiotische Aktivität. Beeinflussungsquoten von über oder von fast 90 % wurden bei den meisten Arten (Staphylokokken, Pseudomonas, E. coli, Enterobacter, Proteus) erreicht; die MHK- bzw. MBK-Werte entsprachen der starken Wirkung. Auch mit der O2-Verbrauchsmessung und dem Tierversuch waren hohe Aktivitäten zu beobachten. Der Vergleich mit den anderen neuen Aminoglykosid-Antibiotika fiel für Sisomicin recht günstig aus — wenngleich auch bei dieser Substanz die individuelle Beeinflussung der Bakterienstämme stets zu berücksichtigen ist.
Summary
The antibacterial activity of the aminoglycoside antibiotic sisomicin was examined in a number of test series. 1280 bacterial strains of different species from clinical specimens were examined with the agar diffusion test. In 200 strains the minimum inhibition concentration (MIC) and minimum bactericidal concentration (MBC) were determined using tobramycin as reference substance. The urine strains obtained during a clinical study before sisomicin treatment were examined in parallel by means of the diffusion and dilution test. The effect of sisomicin was further analysed quantitatively and in terms of time by oxygen consumption measurement under sisomicin influence in five strains each of Staphylococcus aureus, E. coli, Proteus mirabilis and Pseudomonas aeruginosa. Additional investigations concerned the oxygen consumption of six Pseudomonas strains on adding kanamycin, gentamicin, tobramycin, amikacin and sisomicin. The bactericidal effect of kanamycin, gentamicin, tobramycin and sisomicin was determined in the infection and therapy model of acute E. coli or Proteus mirabilis pyelonephritis in the rat. In general, sisomicin revealed high antibiotic activity. A rate of influence of just below or more than 90 % was obtained in most species (Staphylococci, Pseudomonas, E. coli, Enterobacter, Proteus); the MIC and MBC values corresponded with the strong effect. High effectivity was also observed with the oxygen consumption measurement and in animal experiments. The comparison with the other new aminoglycoside antibiotics was very favourable for sisomicin — although with this substance the influencing of individual bacterial strains must always be taken into consideration.
