Summary
Some clinically important β-lactamases are inhibited by flucloxacillin. This ability could be used for protection of β-lactamase susceptible antibiotics. The conditions of β-lactamase inhibition by flucloxacillin were investigated on growing bacteria cultures using the biophotometer. Inactivation was observed by giving flucloxacillin and ticarcillin or cefazolin or mezlocillin separately; flucloxacillin must be added together with the inocula at the start of growth. The concentrations of flucloxacillin required for inactivation are very high and achievable at the most in urine. Subcultures of regrownKlebsiellea pneumoniae cultures were completely resistant to the combination. The sterile filtrate of regrown cultures exhibited no activity of flucloxacillin, but up to 30 U/ml lactamase were detectable. According to the in vitro results the fixed combination of isoxazolylpenicillins with β-lactamase susceptible penicillins is not suitable for therapy of infection caused by β-lactamase producing bacteria.
Zusammenfassung
Durch Flucloxacillin können bestimmte klinisch wichtige β-Lactamasen inaktiviert werden. Diese Fähigkeit könnte therapeutisch zum Schutz β-Lactamase labiler Antibiotika genutzt werden. Die Bedingungen, unter denen es zu einer β-Lactamasen-Inaktivierung kommt, wurden bei sich vermehrenden Bakterienkulturen mit dem Biophotometer untersucht. Zu einer Inaktivierung kommt es nur bei getrennter Gabe von Flucloxacillin und Ticarcillin oder Cefazolin bzw. Mezlocillin; dabei muß Flucloxacillin den Bakterienkulturen von Anfang an zugesetzt werden. Die für die Inaktivierung der β-Lactamasen benötigten Konzentrationen an Flucloxacillin sind sehr hoch und allenfalls im Urin erreichbar. Subkulturen von wiederbewachsenen Klebsiella-pneumoniae-Küvetten erwiesen sich als resistent gegenüber der Kombination. In Sterilfiltraten von wiederbewachsenen Küvetten war keine Flucloxacillin-Aktivität mehr festzustellen, β-Lactamasen konnten bis zu 30 E/ml nachgewiesen werden. Die Fixkombination von Isoxazolylpenicillin und β-Lactamasen labilen Antibiotika ist für die Therapie β-Lactamase produzierender Bakterien nicht geeignet.
