Zusammenfassung
Klinische, mikrobiologische und pharmakokinetische Daten von Studien an 186 Patienten, die mit den neuen antimikrobiellen Substanzen der Chinolongruppe Enoxacin, Pefloxacin, Ciprofloxacin oder Ofloxacin behandelt wurden, werden vorgestellt. Fast alle Patienten waren wegen akuter eitriger Exazerbation einer chronischen Bronchitis, vorwiegend assoziiert mitHaemophilus influenzae, Streptococcus pneumoniae, Branhamella catarrhalis oderPseudomonas aeruginosa, stationär eingewiesen worden. DieH. influenzae- undB. catarrhalis-Stämme waren im allgemeinen sehr sensibel gegen die Chinolone, und die Sputumkonzentrationen von 1,3 bis 4,5 mg/l lagen mindestens um den Faktor 10 höher als die MHK-Werte (geometrische Mittelwerte 0,07 bis 0,44 mg/l). Dagegen warP. aeruginosa etwas weniger empfindlich (geometrische Mittelwerte der MHK 0,4 bis 4,4 mg/l).S. pneumoniae zeigte sich noch weniger sensibel (geometrische Mittelwerte der MHK 0,84 bis 6,7 mg/l); eine Reihe von therapeutischen Mißerfolgen mit diesen Erregern wurde beobachtet. Verschiedene unerwünschte Nebenwirkungen (vorwiegend mit Symptomen des oberen Gastrointestinaltraktes) traten auf, sie waren bei Enoxacin besonders ausgeprägt. Die besten klinischen Ergebnisse wurden mit Ofloxacin, auch bei einmal täglicher Dosierung, erzielt. Die Ergebnisse mit den anderen Chinolonen konnten hingegen nur als mittelmäßig eingestuft werden.
Summary
Clinical, microbiological and pharmacokinetic results are presented from studies in 186 patients treated with the new quinolone antimicrobial agents enoxacin, pefloxacin, ciprofloxacin or ofloxacin. Almost all had been admitted to hospital for acute purulent exacerbations of chronic bronchitis, associated mainly withHaemophilus influenzae, Streptococcus pneumoniae, Branhamella catarrhalis orPseudomonas aeruginosa. TheH. influenzae andB. catarrhalis strains were generally very sensitive to the quinolones and sputum concentrations of 1.3 to 4.5 mg/l exceeded the MICs (geometric mean values 0.07 to 0.44 mg/l) by a factor of more than 10. In contrast,P. aeruginosa was slightly less sensitive (geometric mean MICs 0.4 to 4.4 mg/l) andS. pneumoniae much less so (with geometric mean MICs between 0.84 and 6.7 mg/l) and a number of treatment failures were noted with these organisms. Various unwanted drug effects (mostly upper gastro-intestinal) were seen, particularly with enoxacin. The best clinical results were observed with ofloxacin, even with once daily dosage, but the results with the other quinolones could only be described as moderate.
