Indication and contraindication for surgery of spontaneous intracerebral hematomas

Summary

The indication for surgery in spontaneous intracerebral hematomas is given when, in spite of the administration of dexamethasone in high dosage, an improvement of the initial neurological symptoms could not be observed, or if a worsening is seen. With the help of CT the size of the hematoma could be controlled so that if in comparison to initial findings an increase in mass displacement or herniation is observed, surgery should be performed immediately.

In cases of deep coma with vegetative unstable states, as well as in cases of bleeding into the basal ganglia, surgery is contraindicated.

Zusammenfassung

Eine Operationsindikation zur Entfernung eines spontanen intrazerebralen Hämatoms wird gestellt, wenn trotz der Gabe von Dexamethason in hoher Dosierung keine Besserung der primären neurologischen Symptome oder gar eine Verschlechterung beobachtet wird.

Mit Hilfe der Computertomographie ist es möglich, die Größe eines Hämatoms zu kontrollieren. Im Sinne einer Verlaufskontrolle ist es somit möglich, Verschlechterung in Form von Massenverschiebung, Verlagerung und Einklemmung zu verfolgen und damit die sofortige Operationsindikation stellen zu können. In komatösen Fällen mit vegetativer Instabilität sowie bei Patienten mit rostraler Hirnstammganglien-Apoplexie ist die Operation kontraindiziert.
