Summary
Thirty-two patients with severe pneumonia (22 on assisted ventilation) were entered into a prospective randomised trial, in which fosfomycin plus ampicillin (17 patients) was compared with gentamicin plus ampicillin (15 patients). Treatment was either 4 g fosfomycin or 80 mg gentamicin every 8 h and 1 g ampicillin every 6 h. Complete or partial clinical success was attained in 94% (16/17) in the fosfomycin group and in 80% (12/15) in the gentamicin group. Bacteriological success was 87.5% with fosfomycin-ampicillin and 90% with gentamicin-ampicillin. An intermediary sensitiveKlebsiella pneumoniae strain developed complete resistance in the fosfomycin group, and anin vitro sensitivePseudomonas aeruginosa strain was resistantin vivo in the gentamicin group. Two of three patients in the fosfomycin group receiving the infusion through a peripheral vein developed thrombophlebitis. No other side-effects were observed. We conclude that fosfomycin is at least as effective as gentamicin. Since fosfomycin is widely atoxic and may be given in large doses, irrespective of kidney function, it is considered to have advantages over gentamicin in the combined therapy of pneumonia.
Zusammenfassung
32 Patienten mit schweren Pneumonien (22 davon unter künstlicher Beatmung) wurden in eine prospektive, randomisierte Studie aufgenommen, in der Fosfomycin plus Ampicillin (17 Patienten) mit Gentamicin plus Ampicillin (15 Patienten) verglichen wurde. Die Patienten erhielten entweder 4 g Fosfomycin oder 80 mg Gentamicin alle acht Stunden und 1 g Ampicillin alle sechs Stunden intravenös appliziert. Bei 94% der Patienten, die Fosfomycin-Ampicillin erhielten (16/17) und bei 80% der mit Gentamicin-Ampicillin behandelten Patienten (12/15) trat eine klinische Heilung oder Besserung ein. Ein bakteriologischer Erfolg wurde bei 87,5% der Fosfomycin- und bei 90% der Gentamicin-Gruppe erzielt. Unter der Therapie mit Fosfomycin-Ampicillin wurde ein mäßig empfindlicher Stamm vonKlebsiella pneumoniae vollkommen resistent. In der Gentamicin-Ampicillin-Gruppe sprach einIn vitro empfindlicher Stamm vonPseudomonas aeruginosa nicht auf die Therapie an. Bei zwei von drei Patienten, denen Fosfomycin über eine periphere Vene appliziert wurde, trat eine Thrombophlebitis auf. Andere Nebenwirkungen wurden nicht beobachtet. Wir folgern aus den Ergebnissen der Studie, daß Fosfomycin mindestens ebenso wirksam ist wie Gentamicin. Für die Kombinationstherapie der Pneumonie dürfte das weitgehend atoxische Fosfomycin dem Gentamicin vorzuziehen sein, zumal es unabhängig von der Nierenfunktion in hohen Dosen gegeben werden kann.
