Zusammenfassung

Konzentrationsbestimmungen von Aminoglykosiden im Plasma sollten (ein gut eingerichtetes Laboratoriums-und gut ausgebildetes Personal vorausgesetzt) durchgeführt werden bei Einschränkung der Nierenfunktion höheren Ausmaßes, bei Flüssigkeitsverschiebungen (Ödeme, Dehydration) und bei schweren Anämien. Sind die angeführten Voraussetzungen nicht gegeben, sollte man die Halbwertszeit auf Basis der Kreatininclearance schätzen und daraus die Plasmakonzentration errechnen. Darüber hinaus ist jedoch eine sorgfältige klinische Beobachtung des Patienten unter Aminoglykosidtherapie am wichtigsten.

Summary

Provided a well-equipped laboratory and a properly trained staff are available, plasma concentrations of aminoglycosides should be determined in cases of severe impairment of renal function, changes in the body fluid content (oedema, dehydration), and anemia. In the absence of the above-mentioned facilities, the half-life should be estimated on the basis of creatinine clearance, and the plasma concentration calculated accordingly. Close clinical supervision of patients receiving aminoglycosides is however the most important aspect of treatment.
