New contribution to the problem of connatal viral hepatitis

Summary

The authors suggest that the present definition of connatal viral hepatitis be revised on the basis of the identification of Australia antigen (Hepatitis B antigen). Maternal antigenemia is regarded by the authors as a potential hazard. Furthermore, the probability of displacental transmission on the basis of antigenemia is studied. The probability of connatal contamination is examined in cases of severe serum hepatitis in infancy, in recurrence of serum hepatitis on underlying fibrosis, and in discordant hepatitis of twins. A common feature of the cases dealt with in this paper is antigenemia of the mother and the child.

Zusammenfassung

Die Autoren nehmen an, daß die gegenwärtige Definition der konnatalen Virushepatitis aufgrund der Nachweismöglichkeit des Hepatitis-B-Antigens überdacht werden sollten. Das Vorhandensein von Antigen im mütterlichen Blut wird von den Autoren als eine potentielle Gefahr angesehen. Ferner wurde die Wahrscheinlichkeit der diaplazentaren Übertragung aufgrund der Virämie untersucht. Die Wahrscheinlichkeit einer konnatalen Kontamination wird in Fällen von schweren Serumhepatitiden im Säuglingsalter überprüft, ferner der rezidivierenden Serumhepatitis, die sich auf dem Boden von Fibrose entwikkelte, und der Diskonkordanz bei Zwillingen. Die dominierende Gemeinsamkeit der in dieser Arbeit publizierten Fälle ist die Virämie der Mutter und des Kindes.
