Zusammenfassung
Mit dem passiven Hämolysetest wurde das Auftreten von Lipoid A-Antikörpern beim Menschen untersucht. Parallel dazu wurden die O-Antikörpertiter gegen E. coli, Klebsiella, Proteus und Ps. aeruginosa mit der Methode der indirekten Hämagglutination bestimmt. Es wurden 62 Kinder im Alter von 3 bis 14 Jahren erfaßt, die wegen nicht obstruktiver rezidivierender Harnwegsinfektionen schon über einen längeren Zeitraum in unserer Klinik behandelt wurden. Die serologischen Ergebnisse wurden den radiologischen Befunden bei diesen Kindern gegenübergestellt. Vergleichende Untersuchungen wurden bei 50 gesunden Kindern unter 3 Jahren, bei 50 gesunden Erwachsenen, bei 50 Erwachsenen mit nicht obstruktiven Harnwegsinfektionen und bei 50 Kindern unter 3 Jahren mit bakteriellen Infektionen ohne Beteiligung der Harnwege durchgeführt. Das Auftreten von Lipoid A-Antikörpern ist bei Kindern mit Pyelonephritis statistisch signifikant häufiger als bei den übrigen Gruppen. Bei annähernd 8/9 aller Kinder mit Pyelonephritis konnten Lipoid A-Antikörper nachgewiesen werden. Die Ergebnisse lassen vermuten, daß Lipoid A bei der Pyelonephritis von pathogenetischer Bedeutung sein kann. Die Bestimmung der Lipoid A-Antikörpertiter scheint eine geeignete Methode zu sein, um bei Kindern mit Harnwegsinfektionen eine Beteiligung der oberen Harnwege und des Nierenparenchyms zu erfassen.
Summary
The occurrence of lipid A antibodies in human was investigated using the passive hemolysis test. The O antibody titers of each sample against E. coli, Klebsiella, Proteus and Ps. aeruginosa were also determined by the indirect hemagglutination method. The study was carried out on 62 children aged from 3 to 14 years who were either being examined regularly on a long-term basis or were under-going prolonged outpatient treatment at our hospital due to non-obstructive recurrent urinary tract infection. Serological findings were compared with radiological findings in these children. Serological control investigations were conducted in 50 healthy children under 3 years of age, in 50 healthy adults, in 50 adults with non-obstructive urinary tract infections and in 50 children under 3 years with bacterial infections not involving the urinary tract. The higher incidence of lipid A antibodies in children with pyelonephritis than in children in the other groups is of statistical significance. The presence of lipid A antibodies could be established in nearly 8/9 of all children. The results could be indicative of a connection between lipid A and the pathogenesis of pyelonephritis. The determination of lipid A antibody titers appears to be an appropriate method for detecting involvement of the upper urinary tract and the renal parenchyma in children with urinary tract infections.
