Summary
Thein vitro activity of apalcillin was tested against 350 clinical isolates ofEscherichia coli, Enterobacter, Klebsiella, Proteus, Pseudomonas aeruginosa andStreptococcus faecalis. Using a minimal inhibitory concentration of 16 mg/l as the breakpoint, only 19.7% of the strains were resistant to apalcillin. A regression analysis demonstrated that it is possible to test bacteria for sensitivity with a 20 µg apalcillin disc. The results on Mueller-Hinton agar are very similar to those on Iso-Sensitest medium. Like other ureido penicillins, apalcillin is sensitive to most β-lactamases; it is effective against ampicillin-resistant strains since it penetrates the outer membrane of gram-negative bacteria well and is highly effective against target proteins. Strains producing high amounts of β-lactamases do become resistant to apalcillin.

350 klinische Isolate vonEscherichia coli, Enterobacter, Klebsiella, Proteus, Pseudomonas aeruginosa undStreptococcus faecalis wurdenin vitro auf ihre Empfindlichkeit gegenüber Apalcillin getestet. Wird eine minimale Hemmkonzentration von 16 mg/l als Grenzwert angesehen, so waren nur 19,7% der Stämme Apalcillin-resistent. Eine Regressionsanalyse zeigte, daß ein mit 20 µg Apalcillin beschicktes Testblättchen sich für die Empfindlichkeitsprüfung gut eignet. Ergebnisse auf Mueller-Hinton-Agar und Isosensitest Medium sind sehr ähnlich. Apalcillin ist ähnlich wie andere Ureidopenicilline empfindlich gegenüber den meisten β-Laktamasen, dennoch ist es aufgrund der guten Penetrationsmöglichkeit durch die äußere Membran gramnegativer Bakterien und wegen der hohen Wirksamkeit auf das Target-Protein gegenüber Ampicillin-resistenten Bakterien noch aktiv. Stämme, die große Mengen an β-Laktamasen produzieren, werden auch gegenüber Apalcillin resistent.
