Summary
There are two main periods of manifestation of anaerobic infections in childhood. In the neonatal period anaerobic septicaemia is usually mild with a lower mortality rate, but has essentially the same causes as aerobic septicaemia (premature rupture of the membranes, amniotic infection syndrome after ascending infection). The latter cases comprise about 10% of aerobic septicaemias. Later in childhood, the pathogenesis of anaerobic infections is similar to that in adulthood. Anaerobic infections occur after surgery, particularly of the abdominal region, in malignancies, immunodeficiency and aspiration. With the exception ofBacteroides fragilis, most anaerobes are sensitive to penicillin. A high percentage of clinically important anaerobes are sensitive to clindamycin, chloramphenicol and metronidazole.
Zusammenfassung
Es gibt zwei Schwerpunkte anaerober Infektionen im Kindesalter. In der Neonatalzeit verläuft die anaerobe Sepsis meist leichter und mit geringerer Mortalität, hat aber im wesentlichen dieselben Ursachen wie eine Sepsis mit Aerobiern (vorzeitiger Blasensprung, Amnioninfektionssyndrom nach aszendierender Infektion). Die Inzidenz dürfte ca. 10% der Aerobiersepsis ausmachen. In der späteren Kindheit ist die Pathogenese anaerober Infektionen vergleichbar mit der im Erwachsenenalter. Anaerobierinfektionen finden sich nach chirurgischen Interventionen vor allem im Abdominalbereich, bei malignen Erkrankungen, Immundefizienz und nach Aspirationen. Mit Ausnahme vonBacteroides fragilis sind die meisten Anaerobier Penicillin-empfindlich. Ein hoher Prozentsatz der klinisch relevanten Anaerobier ist empfindlich gegenüber Clindamycin, Chloramphenicol und Metronidazol.
