Zusammenfassung
Cefoxitin hat sich bei der Behandlung schwerer septischer Komplikationen, vor allem nach Perforationen im Magen-Darm-Bereich und gynäkologischen Bereich als eine sichere Therapie bewährt, die im Zusammenhang mit konsequenten chirurgischen Maßnahmen eine hohe Erfolgsquote und niedrige Versagerquote gezeigt hat. Es ist wichtig, vor allem strenge bakteriologische Kontrollen auch während der Therapie fortzuführen, um frühzeitig Superinfektionen durch resistente Hospitalkeime festzustellen und die entsprechenden lokalen und antibiotischen Maßnahmen, vor allem gegenPseudomonas und Enterokokken, zu treffen. Aufgrund der Ergebnisse eignet sich Cefoxitin auch zur perioperativen Kurzzeitprophylaxe bei Laparotomien mit Eröffnung des Dickdarmes.
Summary
In the treatment of serious septic complications, particularly following gastro-intestinal and gynaecological perforation, cefoxitin proved to be a reliable therapy in combination with surgery, resulting in a high success rate and low failure rate. It is important especially to continue strict bacterilogical controls, even during therapy, in order to detect superinfections with resistant hospital-acquired pathogens at an early stage, and to initiate the necessary local and antibiotic treatment, particularly againstPseudomonas and enterococci. The results show that cefoxitin is also suitable for short-term perioperative prophylaxis in laparotomies with opening of the large intestine.
