Zusammenfassung
Bei 12 Dialysepatienten wurde die Pharmakokinetik von Cefoxitin während der Dialyse und im dialysefreien Intervall untersucht. Während der Dialyse betrug die Halbwertszeit 3,5 ± 1,5 Stunden und im dialysefreien Intervall 23,75 ± 7 Stunden. Bei gleichzeitiger chronischer Hepatitis und dialysepflichtiger Niereninsuffizienz ergab sich eine Halbwertszeit von 850 Stunden. Nach diesen Untersuchungen beträgt — gemessen an Dialysepatienten — die nichtrenale Ausscheidung von Cefoxitin etwa 3,5% der verabreichten Dosis. Es ist deshalb mit einer erheblichen Kumulation bei eingeschränkter Nierenfunktion zu rechnen. Im dialysefreien Intervall gewährleistet eine Erhaltungsdosis von nur 0,06 g/8 Stunden denselben Wirkspiegel wie eine übliche Erhaltungsdosis von 2 g/8 Stunden bei normaler Nierenfunktion. Die Erhaltungsdosis während der Dialysebehandlung selbst errechnet sich zu 0,4 g/8 Studen bzw. 0,25 g/5 Std. — je nach Dauer der Dialysebehandlung. Dialysepatienten mit gleichzeitiger chronischer Hepatitis sollten Cefoxitin im allgemeinen nicht erhalten.
Summary
The pharmacokinetics of cefoxitin were studied in 12 dialysis patients during dialysis and in the dialysis-free interval. The half-life of cefoxitin was 3.5 ± 1.5 hours during dialysis, and 23.75 ± 7 hours in the dialysis-free interval. One patient with chronic hepatitis and renal insufficiency necessitating dialysis had a half-life of 850 hours. The results show that the extrarenal excretion of cefoxitin in dialysis patients amounts to about 3.5% of the dosage administered. Considerable cumulation is thus to be expected in patients with impaired renal function. In the dialysis-free interval a maintenance dosage of 0.06 g/8 h results in an effective concentration equal to that obtained with the usual maintenance dosage of 2 g/8 h in normal renal function. The maintenance dosage during the dialysis procedure itself is 0.4 g/8 h or 0.25 g/5 h, according to the duration of dialysis. Cefoxitin should generally not be given to dialysis patients who also have chronic hepatitis.
