Zusammenfassung
Bei 502 Bakterienstämmen wurde die Invitro-Aktivität von Cefoxitin im Vergleich zu Cephalothin im Reihenverdünnungsversuch verglichen. Cefoxitin war bei der überwiegenden Mehrzahl der gramnegativen Stäbchen dem Cephalothin überlegen. Pharmakokinetische Untersuchungen im Cross-over-Versuch wurden bei 17 gesunden erwachsenen Freiwilligen mit Cefoxitin und Cephalothin bei i.v. Injektion und Dauerinfusion durchgeführt. Die mittleren Serumspiegel von Cefoxitin lagen 1/2 und 1 h nach i.v. Injektion von 1 g etwa vierfach höher als die von Cephalothin, während die entsprechenden Serumspiegel am Ende einer vierstündigen Dauerinfusion von 0,166 g/h dreifach höher waren. Die im Steady State gemessenen Cefoxitin-Konzentrationen in Hautblasenflüssigkeit waren dreifach höher als die von Cephalothin (infolge der besseren Diffusionseigenschaften). Die Hautblasenspiegel von Cefoxitin betrugen 77%, die von Cephalothin 54% der gleichzeitig gemessenen Serumspiegel. Die Urin-Recovery von Cefoxitin in 9 h war 89%, von Cephalothin 65%. Cefoxitin wurde langsamer durch die Nieren ausgeschieden als Cephalothin und hatte eine biologische Halbwertszeit von 45 min (Cephalothin von 27 min). Bei Dosisverdoppelung kam es zu einem proportionalen Anstieg der Serumspiegel.
Summary
The in vitro activity of cefoxitin and cephalothin against 502 bacterial strains was compared using the serial dilution test. Cefoxitin was more active than cephalothin against most gram-negative rods. The pharmacokinetics of cefoxitin and cephalothin were investigated in cross-over studies in 17 healthy adult volunteers after i.v. injection and during and after continuous i.v. infusion. Following i.v. injection of 1 g the mean serum concentrations of cefoxitin after 1/2 h and 1 h were four times higher than those of cephalothin, whereas the corresponding serum concentrations after a 4 h continuous infusion of 0.166 g/h were three times higher. Cefoxitin concentrations in skin blister fluid measured in the steady state were three times higher than those of cephalothin (due to better diffusion properties). The skin blister levels of cefoxitin were 77%, those of cephalothin 54% of the serum levels measured at the same time. Urinary recovery of cefoxitin over 9 h was 89%, of cephalothin 65%. Cefoxitin was excreted more slowly through the kidneys than cephalothin and had a biological half-life of 45 min (cephalothin 27 min). Doubling the dose lead to a proportional increase of the serum levels.
