Zusammenfassung
Die antibakterielle Wirkung von Cefoxitin wurde in Routine-Resistenzbestimmungen mit dem Agar-Diffusionstest (Blättchentest) bei 2232 ausschließlich von Krankenhauspatienten frisch isolierten Stämmen der häufigsten als unspezifische Infektionserreger hier auftretenden grampositiven und gramnegativen Bakterienarten untersucht. Im einzelnen handelte es sich um Staphylokokken(Staphylococcus aureus haemolyticus), Enterokokken,Escherichia coli, Klebsiella, Enterobacter, Citrobacter, Proteus mirabilis und indolpositiveProteus species. Vergleichend zu Cefoxitin wurde ihr Resistenzverhalten noch gegen drei weitere Cephalosporine, und zwar Cephalothin, Cefamandol und Cefuroxim sowie gegen Ampicillin, Carbenicillin, Gentamicin, Tobramycin, Tetracyclin und Co-trimoxazol ausgewertet. Cefoxitin ist, wie auch die anderen Cephalosporine, gegen Staphylokokken sehr gut wirksam; gegenüber Enterokokken erweist es sich, wie bekannt, als unwirksam. Bei den gramnegativenEnterobacteriaceae zeigt Cefoxitin eine gute Wirksamkeit gegenE. coli, Klebsiella, P. mirabilis undP. species; als weitgehend resistent erweisen sich hingegenEnterobacter undCitrobacter. Am empfindlichsten gegenüber Cephalosporinen erscheinenE. coli undP. mirabilis; neben Cefoxitin sind die beiden anderen Beta-Laktamase-stabilen Präparate Cefamandol und Cefuroxim gleich gut wirksam. Bei den Klebsiellen zeigt nur noch Cefuroxim gute Wirksamkeit. Sehr unterschiedlich ist die Empfindlichkeit vonEnterobacter undCitrobacter gegen Cephalosporine; meist besteht eine deutliche Wirkungseinbuße.
Summary
The antibacterial activity of cefoxitin was investigated in routine resistance determinations with the agar diffusion test (disc test) on 2,232 strains of/grampositive and gramnegative bacteria occurring as unspecific pathogens, recently isolated only from hospital patients. These strains included staphylococci(Staphylococcus aureus haemolyticus), enterococci,Escherichia coli, Klebsiella, Enterobacter, Citrobacter, Proteus mirabilis and indolpositiveProteus species. The resistance patterns of these strains against three other cephalosporins, cephalothin, cefamandole and cefuroxime, and against ampicillin, carbenicillin, gentamicin, tobramycin, tetracycline and co-trimoxazole were evaluated and compared to those of cefoxitin. Like the other cephalosporins, cefoxitin is very effective against staphylococci; it proved ineffective against enterococci, a fact which is already known. Of the gramnegativeEnterobacteriaceae cefoxitin showed good activity againstE. coli, Klebsiella, P. mirabilis andP. species;Enterobacter andCitrobacter on the other hand were largely resistant.E. coli andP. mirabilis showed the highest degree of sensitivity towards cephalosporins; in addition to cefoxitin, cefamandole and cefuroxime, the two other beta-lactamase stabile antibiotics, were both equally as effective. Only cefuroxime demonstrated good activity againstKlebsiella. The sensitivity ofEnterobacter andCitrobacter towards cephalosporins varies considerably; in most instances there is a distinct decrease in efficacy.
