Zusammenfassung
Bei 26 trachealintubierten Intensivpatienten mit Infektionen des Respirationstraktes wurden vor Therapiebeginn aus Absaugflüssigkeit und Sputum 47 Stämme von 14 verschiedenen Bakterienarten isoliert. Es fanden sich 36 Cefotaxim-empfindliche Stämme, zwei Stämme (1 ×Pneumococcus, 1 ×Haemophilus influenzae) wurden nicht getestet; diese Arten fallen jedoch in das Wirkungsspektrum von Cefotaxim. Neun Stämme (5 von 9Pseudomonas aeruginosa-Stämmen, 1 von 5Enterobacter cloacae-Stämmen und 3 von 3 Enterokokken-Stämmen) waren resistent. Bei acht von zehn Patienten kam es unter Cefotaxim-Therapie zu einer weitgehenden Reduktion der trachealen Keimflora und einer entsprechenden Besserung des klinischen Bildes. Die Ergebnisse der vorliegenden Untersuchung zeigen, daß die Mehrzahl der potentiell pathogenen Keime (außerPseudomonas und Enterokokken) aus dem Respirationstrakt dieser schwerstkranken Patienten mit Atemwegsinfektionen auf einer internistischen Intensivstation gut auf Cefotaxim ansprachen. Somit eignet sich Cefotaxim in Kombination mit einemPseudomonas-wirksamen Präparat gut als Notfall-Therapeutikum bei schweren Infektionen des Respirationstraktes durch opportunistische Krankheitserreger.
Summary
Forty-seven strains of 14 different species of bacteria before antibiotic therapy were isolated from aspiration fluid and sputum of 26 intensive care patients with infections of the respiratory tract and tracheal intubation. Thirty-six strains were sensitive to cefotaxime; two strains (Pneumococcus andHaemophilus influenzae) were not tested. Cefotaxime is, however, effective against these organisms. Nine strains (5 of 9Pseudomonas aeruginosa strains, 1 of 5Enterobacter cloacae strains and 3 of 3 enterococcus strains) were resistant. In eight of ten patients, cefotaxime therapy led to a considerable reduction of the tracheal flora and a corresponding improvement in the clinical condition. The results of this study show that the majority of the potentially pathogenic organisms (exceptPseudomonas and enterococci) from the respiratory tracts of these critically ill patients in a medical intensive care unit respond to cefotaxime. Thus, cefotaxime combined with an antibiotic effective againstPseudomonas is suitable for emergency therapy in serious respiratory tract infections caused by opportunistic pathogens.
