Zusammenfassung
In Routine-Resistenzbestimmungen mit dem Agar-Diffusionstest (Blättchentest) wurde die antibakterielle Wirkung von Cefotaxim im Vergleich zu Cephalotin und den neueren Cephalosporinen Cefamandol, Cefoxitin, Cefuroxim sowie den Penicillinen Ampicillin, Carbenicillin, Mezlocillin und Azlocillin und den Aminoglykosiden Gentamycin und Tobramycin bei 1235 von Krankenhauspatienten isolierten Stämmen der häufigsten grampositiven, vor allem aber gramnegativen Bakterienarten untersucht. Im einzelnen wurden ausgewertet:Staphylococcus aureus, Enterokokken,Escherichia coli, Klebsiella sp.,Enterobacter sp.,Pseudomonas aeruginosa, Proteus mirabilis und indolpositiveProteus sp. Cefotaxim zeigte dabei, besonders im Bereich der gramnegativen Enterobakterien, von allen getesteten Cephalosporinen die größte antibakterielle Aktivität und war beiKlebsiella, Enterobacter undProteus den anderen Präparaten deutlich überlegen. Neu für diese Wirkstoffklasse ist seine teilweise Aktivität auch gegenP. aeruginosa. Hier wird es wahrscheinlich eine Frage der Dosierung sein, um bei klinischer Anwendung entsprechende Erfolge zu erzielen. Cefotaxim dürfte aber schon aufgrund seiner hervorragenden In-vitro-Aktivität in jedem Falle eine Erweiterung der therapeutischen Möglichkeiten erwarten lassen.
Summary
In routine resistance tests using the agar diffusion method (paper disc test), a study was made of the antibacterial effect of cefotaxime in comparison to that of cephalothin and the newer cephalosporins cefamandole, cefoxitin and cefuroxime, the penicillins ampicillin, carbenicillin, mezlocillin and azlocillin, and the aminoglycosides gentamycin and tobramycin on 1235 strains of the most common gram-positive and, in particular, gram-negative bacteria isolated from hospitalized patients. The following strains were evaluated:Staphylococcus aureus, enterococci,Escherichia coli, Klebsiella spp.,Enterobacter spp.,Pseudomonas aeruginosa, Proteus mirabilis and indole-positiveProteus spp. Cefotaxime was the most effective of all the cephalosporins tested, especially against gram-negative enterobacteria. It was also clearly superior againstKlebsiella, Enterobacter andProteus. Cefotaxime was also effective to a certain extent againstP. aeruginosa, a new property for this group of antibiotics. The achievement of similar results in clinical use is probably a question of dosage. However, on account of its superior in vitro activity it can be expected that cefotaxime will be added to the range of antibiotics available for therapy.
