Zusammenfassung
Ungeachtet der Entwicklung neuer und zweifellos nützlicher Präparate hat Erythromycin auch heute seinen festen Platz in der antibiotischen Therapie bakteriell bedingter Krankheiten. Neben seiner guten Wirksamkeit ist das weitgehende Fehlen von Nebenwirkungen hervorzuheben. Die gelegentlich beschriebenen cholestatischen Hepatosen verursachte ein nicht mehr im Handel befindliches Erythromycin-Derivat. Ansonsten fallen Nebenwirkungen bis auf nicht sehr häufige Magen-Darm-Störungen kaum ins Gewicht. Der Indikationsbereich für Erythromycin umfaßt für den praktischen Gebrauch vor allem Infektionen der oberen Luftwege, Otitis media, Lymphadenitis colli, Mykoplasmen-Infektionen u. a. m. Bei Penicillinallergie bietet sich Erythromycin als erprobte Ausweichmöglichkeit an (z. B. bakterielle Angina mit Nachweis von A-Streptokokken). Nachdem feststand, daß rektal verabreichtes Erythromycin mehr als ausreichend hohe Hemmkonzentrationen für die in Frage kommenden Erreger sichert, wurde in Fortführung klinischer Studien anderer Autoren die rektale Erythromycin-Medikation in drei Prüfgruppen nach vorgegebenem Schema an 67 Patienten untersucht. Die klinisch registrierbaren Parameter zeigten bei dem Vergleich der beiden Kollektive hinsichtlich der Wirksamkeit und Nebenwirkungen keine Unterschiede. Entsprechend dem Ergebnis von Vorversuchen zeigten sich allerdings bei einigen Kindern der rektalen Applikationsgruppe lokale Reizzustände, die aber keinen Therapieabbruch erforderten. Die eigenen Erfahrungen mit Erythromycin sind bei Einhaltung der vorgeschriebenen Indikationen nach wie vor gut und unterstützen unsere Auffassung, dieses zuweilen als „veraltet“ bezeichnete Antibiotikum auch weiterhin an seinem Platz zu belassen.
Summary
Despite the development of new and undoubtedly useful products, erythromycin is well established today in the treatment of bacterial infections. In addition to its efficacy, the low incidence of side-effects should be emphasized. The occasional cases of cholestatic hepatosis reported were due to an erythromycin derivative which has since been withdrawn from the market. With the exception of infrequent gastrointestinal disturbances, side-effects are negligible. For all practical purposes, treatment with erythromycin is indicated primarily in infections of the upper respiratory tract, otitis media, cervical lymphadenitis, mycoplasma infections, etc. In patients allergic to penicillin, erythromycin represents an alternative antibiotic of proven value (e. g. with Group A streptococcal tonsillitis). Since rectally administered erythromycin is known to provide higher inhibitory concentrations than necessary for the pathogens concerned, an investigation of rectal erythromycin medications has been conducted as a continuation of clinical studies by other authors. A total of 67 patients, divided into three groups, were investigated. The clinical parameters of the two patient subgroups showed no differences regarding activity and side-effects. As was to be expected from preliminary tests, some cases of local irritation in children due to the rectal medication were observed, but it was not necessary to stop treatment. Our own experience with erythromycin, observing the indications for treatment, remains satisfactory, and provides evidence that this antibiotic — sometimes regarded as „outdated“ — still has a significant role in therapy.
Résumé
Malgré le développement de produits nouveaux et sans doute utiles, l'érythromycine occupe toujours une place incontestable dans l'antibiothérapie des infections bactériennes. Outre son efficacité il faut relever l'absence remarquable d'effets socondaires. Des cas isolés d'hépatose cholestatique furent provoqués par un dérivé de l'érythromycine qui a été retiré du commerce. Par ailleurs, les dérangements gastro-intestinaux sont relativement peu fréquents et d'autres effets secondaires sont pour ainsi dire inexistants. Le champ d'indications de l'érythromycine comprend surtout, dans la pratique, les infections des voies respiratoires supérieures, l'otite moyenne, la lymphadénite cervicale, les infections à mycoplasma etc. En présence d'une allergie à la pénicilline, l'érythromycine offre une alternative parfaitement valable (p. ex. lors d'une angine à streptocoques du groupe A). Or, il a été établi que l'érythromycine administrée par voie rectale assure des concentrations inhibitrices plus que suffisantes pour les germes en question. Nous avons donc continué les recherches cliniques des auteurs prćédents, en étudiant la médication rectale à l'érythromycine chez 67 enfants divisés en trois groupes. Les paramètres cliniques enregistrés des deux catégories de patients ne présentèrent aucune différence au point de vue activite et effets secondaires. Comme on pouvait s'y attendre d'après des essais préliminaires, chez quelques enfants traités par voie rectale on constata cependant une irritation locale, mais il ne fut pas nécessaire d'interrompre le traitement. Notre propre expérience avec l'érythromycine — compte tenu des indications prescrites — a été et reste favorable, et il nous semble qu'il conviendra la laisser cet antibiotique (parfois considéré comme »dépassé«) à la place qui lui correspond.
