Zusammenfassung

Die antibakterielle Aktivität von Moxalactam (LY 127935) wurde an 517 klinischen Isolaten im Vergleich zu Cefotaxim, Cefotiam, Cefuroxim, Cefamandol, Cefoxitin, Gentamicin, Ampicillin und Mezlocillin im Mikrodilutionstest bestimmt. Die Invitro-Aktivität von Moxalactam entsprach weitgehend der des Cefotaxim. BeiEnterobacteriaceae war sie nahezu identisch; etwas bessere Aktivität zeigte Moxalactam nur beiEnterobacter spp., hier vor allem bei gentamicin- und mezlocillinresistenten Stämmen. Bei grampositiven Kokken war Moxalactam deutlich schwächer wirksam. Auch β-lactamasepositiveHaemophilus-influenzae-Isolate wurden schon bei extrem niedrigen Konzentrationen erfaßt. GegenPseudomonas aeruginosa war die Aktivität von Moxalactam nur mäßig, allerdings besser als die des Cefotaxim. Außer beiProteus-undSerratia spp.-Stämmen lagen die bakteriziden Konzentrationen für Moxalactam maximal zwei Verdünnungsstufen über der MHK. Deutliche Inokulumeffekte zeigten sich erst ab einer Einsaatdichte von 107 Keimen/ml.

Summary

The antibacterial activity of moxalactam (LY 127935) against 517 clinical isolates was assessed in comparison to cefotaxime, cefotiam, cefuroxime, cefamandole, cefoxitin, gentamicin, ampicillin and mezlocillin using the microdilution test. The in vitro activity of moxalactam was very similar to that of cefotaxime. It was almost identical againstEnterobacteriaceae; somewhat better activity was seen only againstEnterobacter spp., especially strains resistant to gentamicin and mezlocillin. Moxalactam was clearly less effective against gram-positive cocci. Beta-lactamase-positiveHaemophilus influenzae isolates were inhibited by extremely low concentrations. The activity of moxalactam againstPseudomonas aeruginosa was only moderate, although better than that of cefotaxime. The bactericidal concentrations of moxalactam were maximally two dilution steps above the MIC except in the case ofProteus andSerratia spp. Obvious effects were observed only with an inoculum of 107 organisms/ml or more.
